It should be a spontaneous opening of your heart, revealing all your sins and repenting. You don't need a slokha for this. Simply ask Krishna's forgiveness with your own words, then chant the mahamantra. After all, Krishna doesn't care about slokhas, he just wants you to talk to him and love him.
