Soybeans are the only significant dietary source of genistein; however, the amount of soy foods necessary to meet the body's needs can be difficult to incorporate into today's diet. In Asia, the daily intake can be up to 20 times that of a Western diet.

 

What do soy isoflavones do?

 

1. Soy isoflavones are also referred to as phytoestrogens, a category that also includes lignans found in flax and some whole grains. They appear to weakly bind to estrogen binding sites in cells and have either weak estrogen-like or antiestrogenic activity. As such, there is some, as yet inconclusive, evidence that they may reduce mild menopausal symptoms somewhat and reduce bone loss in peri- and postmenopausal women. They also influence the metabolism of estrogen much like indoles in cruciferous vegetables in a way that may reduce risk of breast cancer. I'll discuss that later.

 

2. Soy isoflavones also appear to reduce risk of heart disease through several mechanisms. They reduce total cholesterol levels and also show some antioxidant activity. They appear to be beneficial to arterial function as well.

 

3. Soy isoflavones protect against cancer. Epidemiological studies show a reduced risk of breast, prostate and colon cancers with a high intake of soy isoflavones. In spite of this population data, there has been an interesting controversy concerning these soy isoflavones and breast cancer. A few basic science and animal studies involving breast cancer cell lines showed cancer cell growth in the presence of these isoflavones and concern arose that eating soy foods might increase risk of breast cancer. Many doctors and nutritionists stopped recommending soy foods for women with or at risk for breast cancer. I am now convinced that there is no substantial evidence that soy intake is associated with increased risk of breast cancer in population studies and, in fact, there are many other ways that soy fights cancer. They interfere with a number of enzymes that promote tumor growth including tyrosine kinases and inhibit the growth of new blood supply in tumors called angiogenesis

 

Another specific way that soy isoflavones, specifically genistein, works against hormone related cancers like breast and prostate is by influencing the metabolism of estrogen. Estrogen is metabolized by the cytochrome p450 enzyme system, the main system for detoxifying substances like hormones, drugs and toxins, to 3 main metabolites - 2-hydroxyestrone, 4-hydroxyestrone and 16 alpha-hydroxyestrone. 16 alpha-hydroxyestrone is a potent, dangerous metabolite that has been shown to stimulate growth of human breast cancer cells, both estrogen-receptor positive and negative. It has about a ten-fold greater estrogenic activity than 2-hydroxyestrone, which is not thought to significantly stimulate cancer cell growth. Women with breast cancer have been shown to have a lower 2:16-hydroxyestrone ratio than those without breast cancer. Genistein increases the metabolism of estrogen into 2-hydroxyestrone, as does indole-3-carbinol found in cruciferous vegetables.

 

How much soy should I eat?

 

To lower cholesterol, I recommend eating at least 25 gm of soy protein a day along with a low saturated fat diet. Eating closer to 50 gm a day is even more effective. The minimum amount of soy protein found to reduce mild menopausal symptoms was 20 gm a day with 35 mg of soy isoflavones, although more seemed to be a bit more effective. An intake of 40 mg of isoflavones was associated with slowing bone loss in perimenopausal women. I generally recommend eating whole soy foods rather than highly refined soy extracts. However, if you use a soy protein powder for some of your soy intake, be sure to use one that has not removed the isoflavones.
