With burning and cramping in her feet, especially worse with excercise or at night.Cold extremities in winter. She has some night time heat, though not night sweats/flushes (almost gone since coming to me). Originally came to see me for neck/shoulder stiffness and pain, again with much improvement. Just had a urinary infection for the second time in a year and never before. Small, slightly rapid pulse. Intermittant insomnia. Dusky tongue, slightly yellow coat. I just gave her tian ma wan becouse it addresses feet, hand and neck cramping, reduce wind, invigorate blood, tonifys blood and yin. I have been treating her locally for the neck pain (gb 20, 21, 39)plus ashi pts. H6, LU7, SI3, LI 4, UB 10, 59, and KI 7, 6. Plus moxa to the neck area since she reports relief with warmth, moxa to gao huang shui for heat at night, moxa KI 1. Any Ideas or suggestions. Also gua sha to the neck area. I like to stick to TCM just like you did, but we can not ignore some very likely signs of insulin resistance/diabetes. Guest guest Posted September 12, 2007 Report Share Posted September 12, 2007 also, I would consider some imbalance in Calcium/Magnesium causing muscle cramps, which would certainly be aggravated by exercise or causing something like Restless Legs Syndrome at night. Another likely possibility is a low back problem or sciatica which can cause blockage of circulation (or nerve impingement) to legs/feet specific to movement or postural position. Hot flashes, confuses, depression, anger, insomnia , night sweats, headaches and dizziness are particularly noted. At this age estrogen and progesterone are probably low. We may ask if she is taking any hormone replacement therapy. Burning and cramping in her feet ( extremities are yang location and belong to SP ), neck cramping. This may be caused from phosphorous deficiency not enough to balance with calcium, in this case, Blood pressure may also be high. There was a suggestion of Blood sugar check. But in this case the chief complain is neck and leg symptoms. We should focus on these points. However, there was no signs of diabetes here. Diabetes symptoms : lethargy, lassitude, dizziness, confusion, sweating, polyuria or overeating, bone steaming heat sensation, itching or insect crawling sensation... Plus or minus some allergies... If night fever without sweating , it is blood deficiency. But How severe, depends on its symptom . It happens only in neck and extremities which interprets into Sp-Blood deficiency. If coughing spell is manifested, is the TAIYIN problem. Treatment will be L7 is enough. If technique is not that well then Sp6 must be added. The neck pain must be clarified on which channel ? UB , GB , or SI? If UB is suspected, K60 is used or combined with SI3. If GB is suspected, G44 is used or combined with SJ17. If SI is suspected, SI3 is used or combined with B62. The problem here probably was a sudden pain, not organ related because there was no organ related symptoms. Ge Gen Tang for shoulder pain is fine ( no organ related ) or Shao Yao Gan Cao Tang would be perfect. I do not expect this take more than 3 treatments, one treatment is almost fine. Good luck !
