I do not have much experience with herbs so I wanted to consult the more knowledged ones here about patents that could be used to help his patient. Main signs and symptoms: 40-year-old female patient (P.) suffered an aneurism two years ago resulting in chronic dull headache located on vertex and occiput which flared up app. four weeks ago due to stress from planning own wedding. P. went for scan, but no finds. P. was prescribed Tramadol and Gabapentin for the headache which only worsened the symptom and also dulled her significantly. Aneurism happened during a timee when p was at a crossroad in her life and took a year of physically straining work at a green house (she is formally trained school teacher) No treatment for emotional trauma suffered during hospitalization for aneurism. Symptoms and pulse were in great progress and in 7 treatments and P had her wedding free of headache. After rigorous testing at a Brain Injury Recovery Center her headaches went back to the level they were at went she first came for treatment and acupuncture now only provides very shortlifed relief. Ps main symptoms apart from the headache which significantly grows worse at night are: Anxiety attacks, depression chronic constipation (headache will improve slightly after defecation). Dry stools (condition from before aneurism). Pain around C7 after whiplash trauma some years earlier Running piglet Qi No appetite -> weightloss Increasing fatigue Difficulty falling a sleep, staying asleep Only few hours of sleep Low Blood pressure (Condition diagnosed but not treated long before aneurism) Sensitivity to sound (now cleared even during headache peaks) Facial color: dull yellow complexion with red guest color on cheeks, forehead BO: P insists on perfume (she keeps very neat even though the effort here worsens hadache) Voice: groaning emotion: frustration -> depression Tongue: Pale body, more pale on sides, yellow rootless coating in kidney area (p smokes), teethmarks corresponding to Lower Jiao P: Thin, wiry, fast lacking St Qi, Shen and not crresponding to season (summer) In comparing the organ positions it it was found that Kid and Liv pos were relatively most deficient. Sp is relatively more deficient than the liv pos and quite wiry. Is she fat or skinny? If either, address that issue, deviation from normal weight is often the first problem. Don't make a big deal of it though, a small move toward normal often helps a lot. Is she getting enough water? Does she or do you consider the dull headache the primary complaint? What points are you treating, how often do you treat and how long do you retain the needles? Have you suggested any herb prescription to her? It seems that there is a new injury due to the Brain Injury test. Did you do a differentiation before and after and were there any changes if you did? How low is the blood pressure? I would initially ask this patient to try a bulk laxative on a daily basis, Metamucil or similar until the bm is easy to pass and continuing the bulk laxative at that dosage, then I would re-differentiate now and then. That could be all the symptoms right there, evacuation is very important. http://www.metamucil.com/ You can tinker with her diet if she tolerates that and eventually get her eating enough vegetable bulk to stop the Metamucil. For herbs I would prescribe something from the Fratkin 8C, 12D or 12H areas. My first thought is An Mien Pian, Liu Wei Di Huang Wan and maybe Sheng Mai San (all three - lots of pills), although you may prefer others in these three groups. Start with the minimum dose (on the bottle) and increase if necessary. I would expect a nearly instantaneous relief of the dry stool, insomnia, appetite and headache with the stool softener and the herbs. She may be startled at the sheer volume of pills, but such is Chinese patent medicine. There is an advantage that you can halve or quarter the dosage without having to break pills or find smaller pills. I hope this helps you. Also please tell me acupuncture points used for her. Probably she had major depression & calming depression & stress is main for reliefing headache. I try not to be alarmist with my patients, but in this case (brain aneurysm at a relatively young age, and continuing headaches), I would consider continuing smoking to be tempting death. Nicotine attacks cerebral vasculature, and the fact that she has continuing & worsening headaches would definintely lead me to suspect other aneurisms that have not yet burst. Should one burst, she could die immediately. She may never have read any articles about this; you may want to do a quick search for a good layman's article and print it out for her. Guest guest Posted August 12, 2006 Report Share Posted August 12, 2006 Dear Thomas Sorensen Hi , It would be good to know what are the points that you have used as well. in the p with aneurysm in the brain I personally recommend no to use GB 20 and Du 14 , or points in the scalp area since they may increase cerebral circulation and cause rupture of the the aneurysm , which we had two case report long before . vertex headache I see very frequently in Qi and Blood deficient and depressed for which earth element tonification will give good results mostly .sp6 st 36 ren12 ren 6 .this will help her to eat very well . to pacify the mind and sedate her so that she shall not use the energy that you are making ;h5.7 p6 to make her optimist h3 is a good choice. to remove the heat from her body and help the tonification li 4 /11 . since there is vascular disorder which is of connective tissue origin you may use LU 9 or Sp 5 . avoid Garlic which may cause bleeding and cold food such as yogurt or milk till tongue changes slowly slowly . frequency of treatment : once a week for 3-4 treatment than increase the interval . Results expected : mostly changes in mood should start in one to 3 treatment , but result is initially not stable till you find the factors influencing the ailment and to prevent resolving the Qi. regards . Guest guest Posted August 12, 2006 Report Share Posted August 12, 2006 Thomas, I would ask you more about the abdominal palpation findings (mu and shu points as well as areas where there is tenderness upon palpation. I would suspect that maybe there is some tenderness at the lower left lower quadrant indicating possible blood stagnation. Would you share more info as to this area of organ functioning as I have found that many signs and symptoms can clear when this area is working properly. It seems almost magical. I follow a lot of Kiiko Matsumoto's ideas on treatment. By the way, I do agree with the xue yin being involved but we can discuss further the points and how to use them to resolve blockages. As for patents that will depend upon what dx but am thinking that something to move the blood and remove stagnation. Please let me know. Thanks. Mike W. Looking through the emails I realized that this problem of my patient's might be less systemic and more Shen-related. So theorizing that her Water/Fire axis had been disrupted by the popped aneurism might have been the main issue creating a block I administered " Internal Dragons " and tested for Agressive Energy and drained what AE I found - It made the pulse image return to a near non-pathological pulse and it made a huge difference in the patient - Headaches as well as depression/frustration lifted almost completely, so I will go along that route and see what it brings.
