Realization is not something to be gained afresh, it is already there (in the words of Ramana Maharshi). It doesn't (necessarily) affect the body/mind directly... rather, it's something else entirely, beyond definition. There are no words for it, there is no answer to your question, so the ancients made up some explanation to satisfy the mind, " prarabdha karma. " There is no karma 'after' Realization, the wheel is departed once and for all. The entire concept of 'karma' is for " those who haven't Realized the Self. " This is the best explanation that can be offered. Don't take the scriptures at literal face value (please). Especially the Upanishads, were given in sit-down 'talks' intended for specific situations and specific people, hundreds of years ago. So were the words of the sages... specific situations. If you investigate, as to " Who am I? " and do what the sages recommend, you will find out... so please don't " take my word for it " either. Investigate and find out, or be content with dream explanations that satisfy the mind.
