Dear Jon, If i'm not mistaken, this is the 'Suvarnamala Stuti' composed by Adi Shankaracharya. It reads: "Saamba Sadashiva Shambhu Shankara Sharanam me' tava charana yugam" search google using term 'suvarnamaala stuti' and u will get links to the text.
