I am not sure about the wording of the following couplet. Can anyone help me with that and its purport and location?

"Dhyan Moolam Gurormurty | Puja Moolam Gurorpadam ||

Mantra Moolam Gurorvakyam | Moksha Moolam Gurukrupa ||

Please inform me whether the wording is correct or not.
