Thanks! I have forwarded it. I am aware of much of

the information and have forms to by-pass the madness.

the Religious based one is procured from the office of

the Superintendent of Public Schools. This is the one

I use to get my granddaughter in schools, though I am

not religious. Recently, I was asked which religion,

I just responded, " Jehovah's Witness. " These keepers

of the gate are serious about requesting vaccination

records because of gross ignorance that they take as

enlightenment. The office worker told me that she had

never seen that kind of form during her seven years at

the school. I assured her that it was legal. I did

have the one from WHO that is in their charter. It

got missing during my last trip to Ghana. I also use

the the documents for myself since I am definitely

afraid for the modern frankenstein Drs. to pierce my

skin in anyway! Also, Dr. Africa has one on page 381

of NUTRICIDE.
