Hello All, I need some advice/suggestions...

 

A 35 year old male with a visual obstruction- he gets dizzy, looses a quadrant of vision (upper left), it gets black/blurriness and dizziness accompanying it. His hands shake.

he has very low cholestrol, Bp normal, no photophobia.

His pulse is moderate (he exercises alot) and his tongue is tender and red//pink with little coat.

Any suggestions for acumoxa and herbs??

I have him on a variation of qijudihuangwan.

It would certainly be nice to have more information on this patient's pulse. What exactly do you mean by moderate, since this can mean either normal or borderline slow. Is it thick or thin, deep or superficial, hard or soft, etc.? And if so, where are the places that are hard and soft, etc.? Does the pulse vary any in speed or quality, as many do?

 

Having said all that, it seems to me from what you have presented that you have at least a basic grasp of the situation, having given a variation of qi ju di huang wan. There does appear to be kidney yin xu with yang rising, and the shaking hands suggest wind, perhaps due to blood xu wind. Or perhaps there is yang rising and going into the upper shaoyin channel (there's a diagnosis I've never heard before) as well as the head. In either case I would suspect the hand shaoyin channel as being affected. If it is blood xu leading to wind, Ht-3, the water point, seems indicated. The heart channel also has three channels which go to the eyes, and if its paired shaoyin kidney channel is vacuous, it could be affecting the hands and eyes due to the heart channel's pathways through both areas. I suggest points such as Kd-3, Kd-6 and Kd-10 to strengthen Kd yin and fluids and bring energy down, and heart channel points as seem appropriate according to diagnosis. The heart luo, divergent and main all go to the eyes. I suggest some herbs for heart blood xu, such as long yan rou, as possibly useful for the eyes in this case, to add in to your formula. Just some fairly random thoughts. Hope they are of some use.

This patient's symptoms sound a lot like "Migraine Equivalent" (actual headache doesn't happen), which would suggest treating Liver/Gall Bladder (I'd use needles rather than moxa: try sedating Liver...what else would depend on more details about the patient). But the shaking hands (in addition to Liver Wind symptoms which might be reflected in the vertigo and visual symptoms) might possibly suggest a hypoglycaemia issue. Any history of this? It would be interesting to do a glucometry during these episodes and see what his blood glucose is like (e.g., below 80?). Having a glucometer handy (especially the new ones (about $50) which permit rapid readings from tiny samples taken with nearly painless autolancets from arms or legs) can be very revealing. Anyhow, just a longshot thought). I'd have to know more before I tried any herbs.
