The worst one is an internally disrupted disc. This is a situation in which the annular fibers are completely disrupted. The nucleolus constantly leaks. When one does a discogram as soon as the medium is injected into the disc (before there is any pressure) the patient feels his or hers pain which is usually sever. These are the patients that have seen everybody and nobody has really helped. The MRI may look OK ie no nerve root or cord compression. They often have HIZ (high intensity zones on the T2 MRI image). The diagnosis can only be done by discogram. There is no affective treatment. The only option (which is not always a good one) is fusion. The root is " toxin " , we should think about, how the disc can move or bent. It must have some reason. Now, for example " Why I lost my money, because some body stole it " so, same reason, somebody must move the disc or somebody must bent it. Who is it? : Toxin. TOXIN is big criminal to cause many problem as back ach, knee pain, heart attach etc . My teacher was very smart lady, she was kungfu lady and know 5 languages. One day the Afghanistan DR was died, before he died, he was afraid that the ten channel will be lost forever. He taught my teacher because he did not have any children. After my teacher going to my country, she begin to treat patients.
