Guest guest
Hi everybody: New patient, just met her, quick overview: Complaint: Has a constant taste of salt/brine in her mouth. No ability to taste anything except spicy - when she puts salt in her mouth she can't taste it. Happened back in July, she was eating some chocolate cake and she realised it didn't taste right. Background: Low back pain / 5 years. Soreness, stiffness. Goes to sleep at midnight. Sleeps with feetout from under covers, comfortable otherwise, doesn't wake. If sleeps during day (fairly regular habit of taking a nap) will wake up feeling cold. " Terrible morning person " , want to be left alone with her coffee (1/day) and cigarettes. Doesn't eat anything for breakfast. Feels that cold and damp weather is " horrid " . She says that she worries a lot. Has had small blue spider veins at both K3 area, left and right, since at least her teens. Tongue Light pink, slightly pale tongue body Slightly yellow coating to rear, med to thin thickness Dryish at centre, some extra moistness at edges Slight scalloping. Had an MRI to investigate the complaint. Nothing abnormal except some very mild ischemia in some areas (lack of blood flow). Woops, no time to take the pulse today. Ok, evrybody, thoughts? Thanks, Hugo ________________________________ Hugo Ramiro http://middlemedicine.wordpress.com http://www.chinesemedicaltherapies.org ________________________________ " Ariel Solomon, L.Ac. Guest guest
Hugo, My two cents..... > Complaint: > Has a constant taste of salt/brine in her mouth. Spleen disease moving to the Kidney - Lifestyle issues (food, activity/rest regulation, not keeping warm enough) The salty taste mimics the functional property of softening tissues and draining water (Yin) to increase fire (Yang) when this function goes awry the body will let you know! > No ability to taste anything except spicy - when she puts salt in her > mouth she can't taste it. > Happened back in July, she was eating some chocolate cake and she > realised it didn't taste right. Spleen not controlling taste (the reaction to the spicy taste points to some Lung imbalance or other) > Background: > Low back pain / 5 years. Soreness, stiffness. Spleen def w. Cold in intestines (UB 25, 27, 28 should be tense, might have pain on palpation, there will most likely be soft cigar shaped lumps (phlegm) extending from the spine and out towards sides - Best treatd with moxabustion), Kidney def w. Cold in Lower Jiao (Entire low back cold - could be tight, could be flaccid, could be damp to the touch) , Lung def w. cold (UB25 should be tense), Blood stagnation due to Cold Phlegm. > Goes to sleep at midnight. Spleen Yang def with Cold - use percussion @ Ren 12: Hollow sound will indicate Yin (Thin mucus) in Stomach/intestines sometimes you will hear water slushing sounds - palpate left Sp9 for pain to confirm > Feels that cold and damp weather is " horrid " . Damp-Cold abundance > She says that she worries a lot. Spleen deficiency > Has had small blue spider veins at both K3 area, left and right, since > at least her teens. Kid def. w. Cold congealing Blood = Blood stagnation propably since onset of menstrual period. There should be Dampness in Lower Jiao which will combine with the Heat of the Large instestine (pain on palpation of UB25, ST25, 26, Kid11, 16) > Tongue > Light pink, slightly pale tongue body Tendency towards Blood def > Slightly yellow coating to rear, med to thin thickness Intestinal Damp Heat > Dryish at centre, Stomach Heat > some extra moistness at edges Points to general edema due to Cold (Sp, Lu and/or Kid) > Slight scalloping. yin / Jing def > Had an MRI to investigate the complaint. Pulse would be nice, but here's a lot to go by :0) Spleen deficiencey w. Cold due to a Sp constitution complicated with lifestyle issues?-> not transforming fluids which sink to the Lower Jiao obstructing the Kid Yang trapping heat in the intestines combining with thin mucus becoming Damp-Heat/ probable congealing of Dampness into Phlegm due to Cold -> some Blood stagnation will eventually follow if it already hasn't. Root treatment could then be PC7, Sp3, 9, Kid 2, 7, 10 (choose from the ones that are painful on palpation) to regulate the Spleen into controlling the Kidneys better (transforming Dampness). Symptomatic treatment could be LI4, SI4, BL64 to release tension/pain from UB 25, 27, 28 respectively and needle any induration found in the lower + moxabustion.
