fee for travel time for housecalls

i have offered to do house calls on a new patient with end stage renal dz

and CA. it turns out he lives 1-1/2h from my office. wondering what to

charge for travel time? would appreciate suggestions.

I generally add $100 to my visit for house calls, but 1 1/2 hrs is a

hefty drive. think about how many patients' fees you'd lose in 3 hrs.

travel time and figure that into your house call fee. that's my

suggestion, anyway.

Wow, Kath. 1.5 hours commute time is alot!  That's 3 hours round-trip.  I'm not

sure what I would charge under those circumstances - I would surely be looking

for benefits other than financial, because that's a huge time commitment that I

wouldn't feel comfortable charging for.  I have traveled up to 45 minutes

(one-way) to see a patient, and then charged an additional $15 total on top of

my usual fees.  That was in the days when gas was much more affordable.  In your

case, you'd be spending 3x as much time traveling, as treating.  Is this time

expense recoverable as fees?  Not all of it, I think.

 

I'd love to hear how you decide to handle this.

I charge $250 for treatments not in my office (usually IVF transfer

treatments). This usually covers any travel expenses and time. At that

price they are usually pretty motivated to get to my office which is my

preference as well. But occasionally it is worth it for the patient and

so I've got to make it worth it for me too.

yes, that's my point as well. on occasion I just charge $100

(dropping my office fee), and that is for either elderly who have no

transportation or someone who is bed-bound, but in both cases they

must live close enough to make it work for me as well (like 15-20

min drive tops).- sami

Chris:

 

i ended up doubling my nml tx fee for the housecall, and charging .505 for

millage (115 total = $58, this is the IRS rate for businesses). it ended up

being around $200, but i wondered if it should have been more, given the 2.5

h of driving time? i'm a little cautious in this case as the patient is at

end stage, & i don't think he's real mobile to be able to do the drive

himself to get to my office. the family is regular folk, not upper middle

class by any means. still, i need to come up with a policy that would apply

to everyone, should another situation arise.

 

your thoughts on charging for the driving time?

 1 hundred dollars is the accepted rate for a house call. If someone is closer,

the treatment less complex and/or ability to pay is a consideration. charge

less. If I get a call from an expensive hotel that have me in their files as an

on call massage / acupuncturist, and it is a wellness visit for someone that

money isn't a big issue, you can charge more.

actually, after talking online to many colleagues, the accepted practice for

house calls is to double the regular tx fee, + mileage. i used the IRS rate

of .505/mile, which is customary. everyone advised charging the regular

hourly rate for the additional driving time. it turned out to be a 3.5-4h

time commitment, so i tripled my usual visit fee + mileage.
