This person has been on prednisone for a long time and has definite heat, deficient heat, yin def. and is basically dried up (def jin ye, etc.), however he also has signs of dampness and phlegm, probably because the prednisone makes his internal environment so nice for a yeast type overgrowth. Tongue is red with thick coat and even large plaques of white, most scrapes off pretty easily. Pulse fast, slippery, empty. Where would one start to treat him? I have figured to really reduce heat and def. heat along with yin nourishing (I think the heat is the root, what he has been treated for in WM too and the prednisone adds more heat and stagnation and now he is yin def and dried up). Hi Brian My best teachers - also very effective practitioners - always say to clear excess first, and once you and the patient see and feel a difference, begin the nourishing. The excess heat will be much easier to clear than the great challenge in a Yin Xu case - co-existing Phlegm or Damp. Your patient will feel so much better just to have some of that heat cleared... calmer, better sleep, less inflammation etc Perhaps use acupuncture to clear excess heat and to gently move a bit of Qi, and then herbs, diet, lifestyle to address the Phlegm/Damp/Empty Heat?
