Now I have a new patient who will require a superbill for reimbursement form her insurance carrier. I got treatment codes down, but what about diagnosis? I'm sure the codes on my form are outdated, and I can't find the codes online. She has pain and swelling between the first two toes of one foot, following a wrenching adjustment from a chiropractor, who forcefully pulled the two toes in opposite directions. Looks to me like a bursitis, but that's not something we LAc's can diagnose. Any hints? Also, any new superbill forms online that I can download or copy? You are not diagnosing per se, but are just letting the insurance company know what you are treating her for. We do it all the time, but only when dealing with insurance. Pain in limb 729.5 (can be used for any pain in any part of any/all limbs - finger, toes, hand, arm, foot, leg, etc) Generalized Pain NOS 780.96 (for general unspecified non-limb related pain) Also, if you have confirmation of a sprain/strain injury (sounds likely from the description of onset), then look in the 800's for the code pertaining to the foot.
