Guest guest
Please offer suggestions and advice for the following case. (I'm a new practitioner and in that doubting phase...) 1st time with acupuncture - can only commit to 3-4 visits due to lack of funds (I am seeing him on a sliding scale to accommodate him). 42 y.o. male, chief complaint ride-sided tinnitus, low pitch but sudden onset starting Aug. 2007. Can go from loud to soft to loud and is intermittant, but occurs 7-10x/week, sometimes for hours per occurrence, or can be constant for days at a time. Worse with spring seasonal allergies, when he also experiences wheezing, sinus dryness and pain. Hx - childhood migraines, mostly ceased at 30 y.o. (family pattern) - Kidney stones, passed one last fall and currently has 6-7 formed according to x-rays. - eczema, especially on head, itchy with scales - dry skin, not dry lips or nails, constantly thirsty for cold drinks - floaters and blurry eyes, especially in morning. - total insomnia 5-6x/month - high stress and anxiety w/ palpitations - can tend to depression ( " but wife would say tends to irritability " ) - high blood pressure, onset 3-4 years, taking Nephedprin - HBP meds causing excess gum growth in mouth - calcium deposits in salivary gland on left side; surgically removed this week - M.D. suspects more forming - occ nausea, especially with acidic fruits - claims to be allergic to fruit, esp apples - runs warm, craves cold drinks, salt and sugar - no low back pain or knee weakness Tongue: pale red body, greenish greasy coat from midbody to root, red tip Pulse: thin, slightly choppy, wiry at LIV, sli slippery at Sp - moderate to sli rapid other: red hair (so pale skin) but red neck and tendency to flush in face while talking So. My first read is that there is phelgm/damp heat in LIV/GB causing stones, nausea, etc. but my typical recommendation to do a LIV/GB flush to support acu treatments usually includes drinking acidic juices/vinegars and eating apples. I am also thinking LIV Yang rising with LIV Bld defic and some HRT fire. I assumed the tinnitus, his chief complaint, is LIV related, but it's low pitched. Of course, with the KID stones, KID could be implicated, but it was sudden onset and there are no other KID defic signs. Since this was his first treatment ever, and he was a little nervous, I did a simple balancing treatment of LI11, LI4/LIV2 (was more tender than LIV3), PC6, LIV5, GB34, and Yin Tang. He reported feeling relaxed and calm, although he had a difficult time settling down while on the table - " had to fight off being fidgeting and itchy " Since I may only get him in for a few more treatments, I'd like to at least have him take a good formula that will begin to address the root issues(I see him socially so can check in on him about the herbs). Thanks in advance for your wisdom. warmly, Julia Carpenter, LAc Berkeley, CA
Guest guest
Hi Julia, Do you still want a formula something for references to tackle your case? It is a complicated one, some signs are easier to be cured, others are not. I do not understand ride-side, I actually had made up a formula for your case but did not post it. The reason is I had replied a few cases like that (5 to 6)before but there were no follow-up whatsoever. I remember one poster told me he would keep me informed. In another case, I wanted to know what happened and sent an email to the poster for feedback, the email add. has been cancelled. How interesting, but it does not bother me much as it is internet. It, however, does take time to analyze the case at my best knowledge. I think your case is worth to know more.
