I have a patient who is coming to me who was requesting Chinese herbs, and I would like to run this by all of you to see what you might have to say. This patient is 24 years old, a male, and a student. He came in seeking an herbal formula. He had been given some herbs by a Tibetan medicine doctor, which he will be running out of in one week. He had thought he had more, but apparently they were running out sooner than he thought. He said these herbs " keep him sane, " so was anxious to keep up with some form of herbs. His chief complaint is Chronic fatigue. His fatigue started about two years ago, after an " ill-advised " candida diet. He was doing a diet of quinoa, vegetables, and some meat. He felt that he had lost weight, and he tried to eat more fat which seemed to help. He had to cut down his hours at work from 40 hours to 30 hours because of fatigue. He tried homeopathics which seemed to make it worse during thetreatment, but would later help. However, sometimes he felt as if hehad low blood sugar after taking the homeopathics. He also explained to me, as a background, that he felt he had a mildform of asperger's syndrome, a mild form of autism. The reason he feltthis was that he had always felt " like an alien " , very different thanothers. He felt a veil between himself and other people, and had ahard time coming up with a response, as if there was a long delay whencommunicating with others. He said the homeopathics helped with the asperger's symptoms. Last October, he said that his mind had started to become affected. His thinking became dull, confused, and he felt foggy headed. He also felt unable to make decisions. It was almost impossible to study for school. He could not concentrate or read anything " more complicated than Harry Potter. " Being around other people helped, as it took his mind away from things. His usual form's of helping himself seemed to make things worse; especially meditation. Two weeks ago he went to see a Naturopath and he diagnosed him with Gilberts syndrome. This is a syndrome where the liver is unable to break down bilirubin. He has had tests for lyme's, lupus, and environmental toxicity, all negative. He tested positive for adrenal fatique, he has low testosterone, as well as a mild thyroid imbalance. He is having nutrient injections, taking anasthatol (a b-vitamin), liver support herbs, bovine granulars and some immune support herbs (mushroom extracts). He is also going to have his Mercury fillings removed next week. Chinese 10 questions: temp: tends to be cold sweats:no pain:none thirt: none particular, drinks water because he knows he should. likes warm drinks digestion: lots of food allergies, some bloating, belching. Had food poisoning two days ago, so stomach is still unsettled. stool: tends towards constipation, but takes pectin, and psyllium husk, as well as digestive enzymes to keep him regular. stool once a day energy: now 5/10, which is good for him. Started feeling better with nutrient injections, herbs emotions: frustrated by his health No heart palps, no dizziness, dryness, headache or dizziness. pulse: both sides were thin, rapid, and slippery-wiry. Not an expert pulse reader yet, pulse felt slightly twangy as if another pulse on top of it, if that makes any sense. tongue: pale, thin white coat in front, yellow coat in back, heart crack. slightly puffy. Chinese diagnosis: Spleen qi xu, with phlegm misting the mind, as well as some heat. treatment: Tonify spleen, resolve phlegm, clear heat. I was confused about the claim of being cold, but pulse and tongue definitely indicate heat, constipation would indicate heat. I thought that the heat might be there from the food poisoning, some kind of invading pathogenic heat. I told him I didn't want to give him a constitutional formula yet, as it seemed he may still have some symptoms relating to his food poisoning. He said he needed to save money so didn't want a separate formula for the possible symptoms relating to food poisoning. I gave him acupuncture instead and he is coming back next week, and would like a formula. Any suggestions or comments? sorry to be so long winded, but I could use some help with this one! Thank you. Guest guest Posted April 10, 2008 Report Share Posted April 10, 2008 i just wanted to note here that you have to be very careful when slapping on a diagnosis of any color on a patient, especially when it is self diagnosed. this does not mean that many patients have a really good sense of what's going on with them, because they do... it only means that you should double check diagnostic criteria before slapping on a diagnosis that could be more harmful than helpful to a patient. here are the dsm criteria for asperger's (note that language delays are not specific to aspergers, nor are any other specific developmental delays. this person may have some sort concentration difficulties, but not necessarily asperger's): http://web.syr.edu/~rjkopp/data/as_diag_list.html ASPERGER's DISORDER (DSM IV Diagnostic Criteria) This document was taken from a post on an Autism related list. This is for informational perposes. DIAGNOSTIC FEATURES The essential features of Asperger's Disorder are: Criterion A. Severe and sustained impairment in social interaction Criterion B. The development of restricted, repetitive patterns of behaviour, interests, and activities Criterion C. The disturbance must cause clinically significant impairment in social, occupational, or other important areas of functioning. Criterion D.
