Guest guest
Hello group. I have a patient 29yo who has had asthma since she was a child. Initially it was excercise induced then became allergy induced. She is on daily steroid inhalers. Her sx are generally controlled unless she has a cold. Plus she has a conjested frog in the throat sensation, has been lactating w/o child for the last two years (bc or no bc), and she had night sweats though these seem to be gone since coming to see me. She also has night terrors around menstration. Easy bruising. Slippery pulses, softer in the lung area, sl. floating in the heart area. Smaller on left. Tongue puffy, thin white coat, pale, sl. wet, red spots at front. red tip. So my main question is: She has been blowing much better #'s since seeing me for the last month. Originally her #'s were 385 and now they are consistantly 450 sometimes 475. But she has had some wheezing she did not have before. At first it seemed cold related so I added gui zhi and that helped when it was cold induced. She did feel like she was fighting a cold which she did not get despite everyone else having which is new. Acupuncture immediatly imeliorates both the wheezing and Lactation. St36, St40, LV3, Cv 17, Sp6, Ki7, P6, Sp4, Lu9, H7, Li4, Du14, Ub13, Yin tang. Moxa to St 36. I have been giving her Bu fei tang jia jian Huang qi 14g Dang shen 14g Wu wei zi 11g zi wan 11g sang bai pi 9g nan sha shen 9g shu di 7g fang feng 7g bai zhu 12g gui zhi 5g mai ya 6g I am thinking of reducing the yin tonics first since her hot flashes are gone. To see if maybe they are just too cloying. Any ideas would be appreciated. Thank you, Petra.
Guest guest
Hi Petra, This kind of asthma, to me it seems to be a Heart fire attacks the Lungs. Let's look at it : Slippery pulses ( damp or phlegm - mostly damp ), softer ( dampness )in the lung area, sl. floating ( Fire or fire element, mild not severe )in the heart area. Smaller ( deficiency )on left. Tongue puffy ( deficiency ) , thin white coat ( no exterior problem ), pale ( normal or deficiency ), sl. wet ( dampness ), red spots ( fire ) at front. red tip ( Heart area or external ). Based on pulses and tongue diagnosis, this is a Heart fire ( mild ) attacked the Lungs caused Lung not disseminated or not distributing the upperjiao ( the fluid ) properly. If you asked for more, your patient might have been experiencing the chest stuffiness, discomfort or distended. There is no exterior invasion from pulses diagnosis. If this is an interior erupted, then we might ask for more details. Because of heart fire, I would ask for sleeping condition ( insomnia or irritability, restlessness, confusion sometimes ), thirst, dry mouth, dry throat or dry stool... Any headache or distended headache ? Any diarrhea ( if not, dampness is mild, we can ignore this kind of damp )? The reason is Heart fire is the cause, not the Lung. In this case, Fire is lowering and Qi is regulating, everything would be fine. This is the Upperjiao problem of excess. If this is excess, it must be Qi. We should look at Qi, not at Blood nor Yin. Now, We are going to investigate the symptoms : A conjested frog in the throat sensation " Plum seed " which is the phlegm stagnated at the throat. I may call it QI stagnation instead of damp or phlegm because it is invisible. We may not cough out nor swallow it......... To disperse this, we need a strong Qi to smash it or disperse it. For all allergies, steroids are used as a final decision. Steroids are from our Kid Yang " adrenal cortex " . Kid Yang is one of the source fire, the source of life and growth, of healthy and disease. If Heart fire does not take care of this issue, the Kid yang must, for the healthy Kid Yang will subdue the Heart fire. If we are aiming at Kid Yang only, we aiming at the branch not the root. This will help, but symptoms or disease will return. You have treated this patient with Gui Zhi, or added with Ma Huang. They are good for symptoms of asthma, but only good for Cold syndrome. You had open her bronchioles and dilated her vessels which allowed to breathe easily, but soon it might be worse, for it is a Fire syndrome. Obviously there is no fire indication, but there was heat. My conclusion, This is a chronic asthma. If it is seasoning, then it is an allergy. We must look at the Kid Yang, Liver and SP-Sto ( digestive system ). If this is all year symptom, I would think or look further into Kid-Yang ( to grasp its Qi from the Lu ), Lu ( to descend its Qi ), Liver ( to smooth its Qi, make sure there is no obstruction which may generate dampness / phlegm ), SP-Sto ( to harmonize and regulate all Qi and Blood ). YOur formula for tonifying Qi, reserving or astringent Qi and fluid, Disperse phlegm............ I do not think this formula for a chronic illness and I expect the disease will return. You may look into it further. > > I have been giving her Bu fei tang jia jian > Huang qi 14g > Dang shen 14g > Wu wei zi 11g > zi wan 11g > sang bai pi 9g > nan sha shen 9g > shu di 7g > fang feng 7g > bai zhu 12g > gui zhi 5g > mai ya 6g > Good luck, Nam Nguyen
