Stubborn acne with hormone problems and PCOS

Dear Colleagues,

Can anyone help with the following:

Female 29 years old, long history of PCOS and endometriosis both

controlled very well using vegan diet. Main problem acne on face,

papules and macules, Yang Ming (with phlegm) and Lung heat type

distribution. Initially responded to:

 

Gui Pi Tang, san leng, e zhu, tian nan xing, yan hu suo, yi mu cao,

xiang fu, ban xia, zao jiao ci, bai hua she she cao, bai jiang cao,

zhe bei mu, pu gong yong, zi hua di ding, huang qin, xia ku cao, hai

zao, hai dai, bai zhi.

 

Cleared for two weeks but then back to original problem and been very

stubborn ever since, endometriosis and PCOS responding well to herbs,

skin not. Skin usually clears for one week prior to periods. which

are slightly longer than usual.

 

Pulse: thready, sl slippery Tongue: Purple, sl white greasy coat with

red edges.

 

Help I'm stuck! I've tried everything I can think of your experiences

appreciated. Please only herbs since I don't do acu!

I have had really good results with some of Giovanni

Maciocias formula protocols. There is an article in

Herbal Crossroads, April 2005, by Kan herb Co. on

PCOS. I have used the protocol for pcos patients and

have had great results in getting rid of their acne in

addition to the pcos.

Dear Lorraine,

 

I had come across a few serious cases of acne, but none was similar

to your cases. Somehow, you may look at my case and make a judgment

for your own.

A male patient, age 24, had serious acne for 12 years and have

antibiotics treatment daily. It was so serious that he could not touch

his face because of pain. All pimples were filled with pus and

odorous. I could smell it from 20ft away. The pimples were large and

filled up the whole face. It looked horrible, as a massage sandle. I

had never seen a second one like this.

This patient had a very bad breath ( may be from medication ), dry

skin, red itchy face. He could only splash water into his face, not

touching. Stayed away from all kinds of food recommended by his

dermatologist. Normal bowel movements, not cared about hot or cold

drinks. He was physically strong, healthy and clear mind, but he

tended to get angry easily . Thick Yellow dry coat, full slippery

pulses almost wiry.

After I diagnosed him and assured him that the treatment would

definitely have results within 5 days.

I think most of us would diagnose this as phlegm heat or dampheat

and give all kind of dampheat formula such as ( Ba Zheng Wan or Long

Dan Xie Gan Wan ).

I looked at it in different view. A chronic acne was treated with

antibiotics and not resolving the problem. If I viewed it as dampheat

or phlegm heat, I could have repeated the same treatment.

To me I thought the acne had been there for a long time and became

chronic and an internal illness. The internal heat may be the cause of

antibiotics " phlegm heat , not dampheat because the tongue was not

wet. Besides there was no heaviness sensation at all, no sweating... "

Pulses of full and slippery or wiry, I think because the patient was

robust and healthy no Jing ,essence or Qi and Blood deficiency.

The tendency to get angry easily I thought because the internal

body was too dry ( from antibiotics ), body fluids, Blood circulated

very bad. This circulation could easily cause those fluids became

toxic and caused acne.

ALl I had done is aiming at QI !!!!!!!!!!!!!!!

Regulated Qi. If Qi is healthy and moving properly without

obstruction, I think we are all fine. In order to regulate Qi, I must

tonify it and regulate it. To tonify Qi , it best to tonify Blood

because I think blood will slow it down and moistures it and help it

to deliver its nutrient to all body cells or organs and channels.

The formula was given was " Ba zhen Wan " , 30 pills each, 2x/day.

Surprisingly, 30 minutes later, he told me he could touch his face.

Next morning, he could rub his face. The third day, all pimples were

down 80%, and the 5th day were all gone.

 

NOTE : When I asked this patient to give me a chance to treat him.

He laughed at me with a despited look on his face, " I have been had

my case treated for more than 12 years and I do not believe anybody

who can treat this case. All my doctors said this is incurable, just

slow it down. And now you said that you can treat this within 5 days ?

Get lost ! " Eventhough he said so, he came to see in the next 3 days.

 

After 5 days, his face was completely clear, but the back and the

front shoulders was not cleared " the skin , it looked as bad as a

carpet " . The next formula was give " Jin Gui Shen QI Wan " 30 pills

in the morning, and " Shi Suan Da Bu " 30 pills in the evening. 3 days

later, it was cleared. He continued 2 more days.

The total days of treatments was 10, for face and shoulder. It

was a miracle. It was not me who treated him. I thought it was the one

behind the mystery. But this is the one special case. Please search

for more and ask yourself why it did the healing.

I used TCM analysis, but I looked at different angle. Should

anyone of us take a look at different angle instead of following the

crowd ?

Talk with your patient about their diet and lifestyle. In working with

patients with PCOS, diet is huge. If they are consuming even the smallest

quantities of dairy, refined flour, sugar, coffee, and chocolate then their

acne symptoms will likely not budge. Once the dampness has been cleared then

they can handle those foods in minimal amounts. Having them on a diet heavy

in brown rice, oats, garlic, onions, and radishes will help clear the skin,

too. You may be doing everything right, but the diet is the best medicine

for any Spleen disorder.

Low glycemic index is key for PCOS too. Takes at least a year for

the menstrual cylce to regulate, but I believe this dietary changes

are a must--herbs and acupuncture are not enough. Similar in that

way to diabetes and metabolic syndrome.

lorraine:

 

i missed the orginial post about this, but according to dermatology in tcm

(the big yellow derm book), acne that flares in accordance with the

menstrual cycle is due to disregulation of the chong and ren. the book goes

on to give formulas and point prescriptions. i've been pleased with results

when i've followed the protocals given in the book.

It seems I don't have this book could you outline some of

this for me? I'm trying to get as much info as possible before I go

further. Main formulas would be great with brief pattern description.

it's a very comprehensive book. i find it quite dependable and useful for
