The pharmaceuticals mass marketing campaigns and an aging population don't tell the whole story. The drug consumption is part and parcel of the staggering consumer consumption and the competition to survive as American cultural values erode and the wealth disparities widen. I would add one more thing: Combatting the psychology of fear, particularly fear of pain, and fear of the unknown, are perhaps the greatest obstacles we face in getting started with patients.
