Health Insurance Claim Form

Hi all, does anyone by chance have a copy of the new health insurance

claim form that has an area for the national provider ID? If there's

any way that someone could send a copy my way I sure would appreciate

it. You can either email me at: turningpointacupuncture

Laura,

 

I will do you one better. I will attach one that is fillable and you can save

for use on your laptop. I would say that you will need to whiteout the

workcomp address before you send it off to insr. If someone has found

a fillable pdf that has no insr info, I hope they will post it. Always make

two copies of all claim forms, one for insr and one for reference. Mike W.

Mike and Gary,

 

Thank you!! You both sent me the form, and I really appreciate both

of you taking the time to send it to me. My acupuncture friends will

benefit as well, so thank you!!

 

Anyone know if insurance companies are refusing payments on bills sent

with the old form and no NPI number on it? I did call and give most

of them my number, but haven't been putting the number on the form.

I'd hate to have to re-do my billing for all of June and July.

 

Also, anyone have a program that they love? I've been doing it by

hand in a pretty easy way (I photocopy each time, and just write in

dates), but now that I'm going to have to re-do all of my patients

anyway I may as well get on an electronic system.

I just started using a product by little guy software

(littleguysoftware.com). It is essentially a Microsoft Word template

that someone has created. I can save a copy of each client's form and

simply fill in the new dates and print. The 'software' cost between

$50-70.

if you bill with BCBS you can use payerpath.com (carefirst direct) which is

an online submitting

it is free as long as you dont bill over 250 claims a month

not sure if other companies can use this or not

worth a call to blue cross to discuss with them
