I would say that you will need to whiteout the workcomp address before you send it off to insr. If someone has found a fillable pdf that has no insr info, I hope they will post it. Always make two copies of all claim forms, one for insr and one for reference. Mike W. My acupuncture friends will benefit as well, so thank you!! Anyone know if insurance companies are refusing payments on bills sent with the old form and no NPI number on it? I did call and give most of them my number, but haven't been putting the number on the form. I'd hate to have to re-do my billing for all of June and July. Also, anyone have a program that they love? I've been doing it by hand in a pretty easy way (I photocopy each time, and just write in dates), but now that I'm going to have to re-do all of my patients anyway I may as well get on an electronic system. It is essentially a Microsoft Word template that someone has created. I can save a copy of each client's form and simply fill in the new dates and print.
