Guest guest
Hello, Group I need your feedback I've got a new patient, male 48 y.o.. His Western d/s - " primary hypothyroidism, asthma, scoliosis. Secondary d/s: schizoaffective disorder " . His meds are Leodon, Depalcote, Trihexyphenidyl, Zyprexa, Xanax, Levothyroxine, Lipitor, Ovar, Colace, Lactulose, Gentifer, Albutrol, Mylanta, Benadryl, Tylenol, PeptoBismuth...(and a few others he did not remeber). Many years he had been taking Lithium (not now).He has left eye palsy. Hands tremble. He lives in the assisted living home. His chief complain is pain when he speaks, whispers or whistles. Pain started suddenly six years ago when he was visiting the bird pavilion in the ZOO. (No allergies to the birds). No emotional cause. Five months ago he stopped talking. He writes. Pain is not in the lips. He wrote me that it is hard to explain where is the pain, but it is in the mouth area. Not inside of the mouth. Pain is 8 on a scale from 1-10. He has no pain when he swallows, coughs, chews, sneezes, laughs or when moves the lips as if he wants to make certain sounds. He said (later during the treatment) that people do not believe him that he has pain and that they explain his problem that he wants to get more attention. He went to 3 ENTs, did few MRIs, CATscans, neurologists, psychiatrists , kinesiologist, dentists, speech pathologist with no results. He has no tender points around the mouth. His tongue is with red tip and sides, thick fur yellowish at the root. Pulse is forceful and rolling at left cun and very weak and disappearing at other positions. He did not give me any digestive complains (however some meds he takes show he has them). Nocturia. No palpitation. I could feel the hot air coming from his nose when he was breathing. NO cold hands and feet. Prefers cold water to drink. Sometimes he has migraines. Acne. He started speaking at the end of the treatment, probably because he relaxed and started trusting me. I palpated the points on the tips of the fingers (Su Jok) corresponding the mouth and he had a lot of pain. But he had tenderness in the whole area corresponding to the head. I palpated Mouth area in the ear and it was very tender too. Other points like Kd, Lv, Ht were not sensitive at all. My impression is very mixed. He takes to much medicine. I believe that his heat is at least by half comes from it. I used a very simple protocol LI4(l), LI11®, Ht8®, St40, Pc5(l),Lv3® , Kd3(l), Du20 for 20 min and Du 26 for 1 min. I was not sure how he will tolerate acupuncture (he wrote that he cannot take any sensation of pain at all) and it was fine. At the end I decided to needle Du26 (!). And he said that he did not feel anything (and I started manipulating the needle to get de Qi there with - no sensation). At this point I guess I have problem to understand how much of his pain comes from medicine he takes and from his mental condition. May be you have an idea. Is it his mind manipulates him and he has a kind of psychosomatic pain? Or is it a real pain that comes from obstruction of the vessels by heat/fire and phlegm and/or difficiency of Qi and blood? I do not see the signs of hypothyroidism --May be he needs to cut down the dosage of Levothyroxine? Of course I do understand that it is not mine or his decision to do that. I can not give him any herbs since he is in assisted living and probably I wouldn't because of the amount of the meds. Did you have such patients, what did you do? How do you address the side effects from medicine? With respect, Tatiana
