Letter of Medical Necessity

Hi Group. I have to write my first " Letter of Medical Necessity " ,

never wrote one before and would like to look at some samples, get

information on what to include or not, experiences others have had -

good or bad. I'd like to do this right. Ay help you can give me

would be greatly appreciated. I did look up " Writing letters of

medical necessity " but found very little that helps an acupuncturist.
