Female, 46 years old presented with idiopathic Spinal Muscular Atrophy which was diagnosed 2 years ago. She has weak legs with a particularly weak right hip and has difficulty raising her right leg to walk. Muscle tone is reasonable and there is little wasting. She has no pain other than occasional back ache. There is no noted structural pathology and sensory and motor nerves seem healthy. She has occasional alternation constipation and diarrhoea and has eaten no breakfast most of her adult life. Health history - cervical cancer 17 years ago treated with radiotherapy that stopped her periods. Her feet get cold easily with the nails turning blue.
