Hi Group,

a good friend of mine recently developed polycythemia vera.

whats the tcm pattern of this disease? herbal recommendations?

Acupuncture point prescription?

signs presented were pale tongue,slightly purple sides, skin dry with

small areas of bruising, deficient pulse on the right middle position.

I would friends with polycytemia vera (PV) to seek western treatment,

and possibly supplement that with TCM. I can find little of use re

acupuncture or herbal medicine in PV on Google.

 

Blood-letting is one of the main western treatments in PV (and in

hemochromatosis). That should be done under hospital supervision

only.

They recommend a formula with Jixueteng, Qingdai, Longdancao,

Dahuang, Huangqin, Shuizhi, Taoren, Ezhu and Mudanpi. However,

TCM would use the total S & Ss and try to address the Pattern (Branch)

and its likely Root.

 

Itch is a common symptom in PV and can be addressed herbally and/or

by AP.

 

If right middle deep pulse (SP) is weak, I would include SP02 (Mother

Point) in the acupuncture selection. I would also add SP10 (Blood Sea),

LV03 and BL23 in the list.

Hi Voltaire,

 

you might want to take a look at the book " A Handbook of Chinese

hematology " by Simon Becker. There is a 12-page description of

polycythemia vera (PV) and its Chinese medical treatment in it.

 

I found many articles in Chinese on the subject. There seem to be some

experienced doctors that have developed their own valuable methods in

dealing with this serious disease. Please see a list of all the

articles I found at the bottom of this post.

 

I haven't had the time to thoroughly read through all these articles

and I am far from an expert in treating hematological diseases, but

here's what I got from glancing over the articles. Perhaps they can

inspire you to create your own herbal formula for your patient,

hopefully with good results.

 

Xia Hai-Zhou emphasizes the use of phlegm-transforming and

cough-surpressing herbs, because of their saponin contents, which have

a reputed certain hemolytic effect (hemolysis should be a main

treatment goal, no matter what the pattern). He created the following

formula: zi wan 12, zhu ya zao (zao jia, Fructus Gleditsiae Sinensis)

6, huang qi 24, ge gen 9, bai jie zi 12, yuan zhi 9, ji xue teng 24,

shan yao 12, sheng ma 3'chai hu 6, nu zhen zi 15, bai he 12, zhu ru

15, Zao Xiu (Rhizoma Paris polyphylla) 9, bai hua she she cao 15, ma

chi xian 15, san qi fen 3

The author chooses from some 17 treatment methods, and uses about 91

herbs, but he believes the above treatment method to be the most

valuable in the treatment of PV.

 

Many articles describe common patterns in PV:

- blood stasis collecting in the interior

- [effulgent liver gallbladder] fire

- evil heat entering the blood; heat toxins

- phlegm turbidity obstructing the interior

- insufficiency of Qi and Blood

- kidney and liver yin vacuity

 

Some formulas:

- hua yu xiao zheng tang (transform stasis and disperse concretions):

tao ren, hong hua, dang gui, chuan xiong, chi shao, qing dai, tu bie

chong, xiang fu, yu yin each 10, dan shen, ji xue teng each 30, san

qi, e zhu each 6, shui zhi 5.

 

-another similar formula: san qi 10, e zhu 10, quan xie 6, bie jia 10,

tao ren 10, hong hua 10, dang gui 15, chi shao 10, chuan xiong 12, ji

xue teng 30 with modifications according to pattern: lumps under the

ribs add long chi & mu li, xia ku cao or da huang bie chong wan;

spontaneous bleeding of the flesh (ji nu) add xian he cao, zi cao,

cang bai; bloody stools add ce bai tan (charred arborvitae leaf), san

qi fen, da huang fen; bloody urine add da ji, xiao ji, bai mao gen;

lack of strength add sheng huang qi, dang shen, bai zhu; irregular

menstruation add yi mu cao, xiang fu, sheng di, shu di; exuberant heat

toxin add sheng shi gao, zhi mu, dan pi, shui nu jiao etc.; phlegm

heat add ban xia, dan xing, fu ling, gua lou; liver gallbladder fire

effulgence add long dan cao, zhi zi, huang qin; high fever clouded

spirit add an gong niu huang wan.

 

- ge xia zhu yu tang

 

- long dan xie gan tang

 

- blood moving herbs and qing dai (indigo) are widely used in may articles

 

- formula that cured three cases of PV caused by heat toxing binding

together and qi stagnation and blood stasis: qing dai 20, gan chan pi

(dry toad skin) 20, bai ying (Herba Solani Lyrati )30, shan ci gu 20,

zhu ling 30, bai hua she she cao, 30, huang qin 15, tao ren 20, hong

hua 6, dang gui 15, shui zhi 20, chi shao 30. All three patients cured

in 30-60 days of administration of formula.

 

There is one report which mentions that a large dose of 120g of da ji

( Cirsium Japonicum) was the crucial factor in curing a patient. The

formula had effect only when da ji was added to it: sheng di 15, bai

shao 15, nu zhen zi 12, han lian cao 15, dan pi, 15, dan shen 15, tao

ren 10, hong hua 6, yu jin 12, tu bie chong 12, zhu ye 6, huan lian 6,

pu gong ying 15, tai zi shen 30, mai men dong 30, shui zhi 10. Later

120g of da ji was added to this formula, which brought about a cure in

two weeks (no more symptoms, no recurrence).
