ISO I don't want to immunize my child form

I asked for a form from my son's pediatrician office. They faxed it to me.

I hate the wording; it says I'm endangering the life of my child & other children in contact

with my child.

 

No way am I signing this form.

 

Does anyone know of a form that does not state things this way & is legal in all 50 US states?

May look into trying the religous exemption route.
