In my experience it just confuses things and leads to mistreatment. That Wen Jing Tang helped a lot in the past is important. Cold stagnating the blood can cause the hormonal imbalances. To me the tongue is also key. If it is pale then I would not worry about the acne/heat at all. This heat is coming from Qi depression and the root of the Qi depression is Cold. When you warm, the Qi will open and flow and the heat will naturally diminish. Wen Jing Tang also includes Mu Dan Pi for just this kind of depressive heat. I would put her back on Wen Jing Tang and add Cang Zhu for " bubble like nodes " , Zao Jiao Ci to open cysts, Shan Zha for meat like stasis lumps (endometriosis) as well as Ba Ji Tian and Xian Mao.
