In prevention, CM can treat many common risk factors for stroke, like hypertension & diabetes. In Rehab, CM can treat effects of stroke, like paralysis, speech issues, muscle weakness/flaccidity, etc. In CM theory, stroke relates to LV, SP & Kidney systems. Predisposing factors for stroke may take years to develop & often are dt emotional & physical strain, overwork, poor diet & lack of relaxation. These lifestyle habits deplete body vitality, which often causes Internal factors of Phlegm & / Wind Accumulation, which may culminate in a stroke. Phlegm is dt SP Xu dt poor diet & / physical/mental strain. Phlegm Accumulation may disrupt smooth Qi flow w symptoms, like poor concentration/muddled thinking, & / limb numbness. In time, Phlegm Stag may occur & transform into Phlegm Heat Rising to the head, ultimately causing a stroke. Wind is often dt emotional & mental strain + lack of relaxation & poor dietary habits. Excess stress can deplete both KI Yin & LV Yin, which can cause Wind Rising w a stroke or symptoms, like hypertension, headaches, emotional issues, etc. TCM Tx theories for stroke have two main categories: (a) those affecting muscles/Channels (generally mild) & (b) those affecting Internal organs (more serious). Internal Channel differentiations are further subdivided into: (a) general Xu & (b) general Shi Patterns. Often, patients have a mix of Xu & Shi symptoms. Also, in Rehab of more severe strokes, patients are treated according to the muscles/Channels differentiations which deal w side effects of stroke. TCM Diagnoses & AP Tx • Differentiation: o Zangfu (Internal Organs) Affection (more serious/acute stage): Xu (Flaccid, Empty, Weak) Condition Shi (Tense, Full) Condition o Muscles/Channels Affection (less serious/Rehab stage): Symptomatic Tx • Internal Organs Xu (Flaccid, Empty, Weak) Condition o S & Ss: Coma & unconsciousness~sudden w flaccid signs, like closed eyes, open mouth, sweat~mild on forehead/face, loss of bowel/urinary functions, limbs cold; Tongue: Swollen, maybe Pale; Pulse: Minute o Tx Pts: Valaskatagis Pt Selection: General Tx Pts - Return consciousness, Free Channels & Hypotensor: GV26 - Return consciousness LI04 - Clear Wind from head PC06 - Calm Spasm/Free Channel SP06 - Calm Spasm/Free Channel (possibly w/moxa) LV03 - Clear LV Wind ST36 - Tone Qi (possibly w/moxa) LI11 - Clear Heat KI03 - Tone KI Yin Pts to Tone Xu (Flaccid) Condition: CV04 - Tone Qi/Yang (w/moxa) CV06 - Tone Qi/Yang (w/moxa) CV08 - Tone Qi/Yang (w/moxa) Maciocia Pt Additions: GV04 (Mingmen, Life Gate) - Tone Qi/Yang (w/moxa) BL23 (KI Shu) - Tone Qi/Yang (w/moxa) Internal Organs Shi (Tense) Condition o S & Ss: Coma & unconsciousness~sudden w tense signs, like clenching of the mouth & / hands, redness of head/face, no loss of bowel or urinary functions, breathing course; Tongue: Red w/sticky yellow coat; Pulse: Slippery, possibly wiry o Tx Pts: Valaskatagis Pt Selection: General Tx Pts to Revive consciousness, Open Channels & Hypotensor: GV26 - Return consciousness LI04 - Clear Wind from head PC06 - Calm Spasm/Free Channel SP06 - Calm Spasm/Free Channel (possibly w/moxa) LV03 - Clear LV Wind ST36 - Strengthen Qi (possibly w/moxa) LI11 - Clear Heat KI03 - Tone KI Yin Pts for Shi (Tense) Condition: GV20 - Clear Mind / Shen KI01 - Clear Shi in upper body/head A_01-Shixuan - Return consciousness May also bleed Jing-Well Pts to Clear Heat & Wind~Internal Maciocia Pt Additions: GV16 - Clear Wind~Internal GB20 - Clear Wind~Internal PC08 - Clear Heat ST40 - Clear Phlegm Symptomatic Tx - Muscles/Channels (Rehabilitative) o S & Ss: Stroke may have various side effects; those often treated w AP are: speech problems (aphasia), facial paralysis, & muscular paralysis/weakness (hemiplagia, rigidity). In Rehab, Tx existing underlying factors, like hypertension, Clear Wind & Aid smooth flow of Qi & Xue & choose Pts for local issues. o Tx Pts: Valaskatagis Pt Selection (w modifications): General Tx Pts - Clear Wind, Tone & smooth Qi & Xue Flow: GV26 - Return consciousness GV20 - Clear Wind from head BL07 - Clear Wind from head LV03 - Clear LV Wind ST36 - Strengthen Qi (possibly w/moxa) KI03 - Tone KI Yin Pts for affected area: Choose appropriate Pts on the affected side/limb, largely (but not exclusively) Yang Channel Pts.
