First we should know we have 2 types of tonsils or lymphatic Tissues in throat ; 2 pharyngeal tonsils we see them in back of Tongue & adenoid tissue which is in back of nose , not seen Without special mirror. Pharyngeal tonsils sometimes became Large & cause obstruction to breath & swallow. When they Infect became large & red with white coating(pus) , most dangerous Cause is streptococcal infection which especially in age 3-30 years Causes rheumatic heart disease. So with respect to TCM , I should Say pharyngeal infection should be treated with antibiotics if it is Streptococcal. .TCM measures can act as pain relieving & comforting Agent NOT main therapy.there is no indication for surgery except Pharyngeal abscess or tumor. But your patient has signs & symptoms of recurrent sinusitis.Sinuses Are parapharyngeal chambers filled with air & mucus , their orifice Are in mid of their inner wall so drainage of their secretion are related To constant motion of mucosal cilia .these fine cilia paralysed with Smoking even passive, air pollution , pollens etc. An allergic basis will aggravate secretions in nose , sinuses & throat & impaired drainage ends to a vicious cycle of infection , now Patient have fever sticky yellow to green secretions , sweating & Cough especially at night when secretions go to lung directly . Ear infection is a complication as anorexia & mouth malodour. In this patient you should first pursuit underlying condition. It may be due to stress , HSV recurrence , malnutrition , low sun Exposure , air pollution or smoking . Also consider 2 severe condition: HIV infection & Neoplasm of nasopharynx which should be considered in every ear infection in adults , not children. I suggest to you to have a consult with an ENT to have a blood exam ( complete blood count , ESR ,HIV exam ) & a throughout exam of Nasopharynx with special mirror.Then he should prescribe antibiotics For 2-3 weeks not less ,even symptoms cured in first days.Early antibiotic Cessation is the main cause of recurrency. Don`t be worry I am not an enemy of TCM , but I think it should complete Classic medicine.What is the rule of TCM ? You can prescribe herbal drugs for mucolysis , increase immunity , lessen Cough & soften respiratory secretions .they acts very nice. In other hand you should repair the underlying condition , if it isn`t HIV Or tumor ( they chance is less than 5 %), it is allergy & depression. In TCM diagnosis we can say he had a week wei qi , deficient lung qi & cold invading to him .So you should repair them . I had many patient Like him even in children treated with acupuncture better than drugs. Anti allergic drugs cause somnolence , drying secretions & transient Effects. I suggest you needle these points : For general increase in wei qi: Li 4, 11 , Du 20 , K 3 ,Sp 6 For increasing lung qi : Lu 1 , 6,7 , 9 ,UB 11 , 12 , 13 ,Sj 5 ,St 40 For increasing qi in face & sinuses: St. 3 , 8 ,10 , 11 , 12 ,Li 19, 20 Yuyao,yintang , SI 16 , 17 ,UB 2 , 3 , 4 , GB 14 For depression : Du 20 , Li 4 , 11 ,LIV 2,3 ,H 7, P6 Moxibustion also may be useful. Also give him some face Yuga exercises. And also refer him to an infectious disease expert for acyclovir Suppression therapy for reduction of herpes relapses. If any other idea or need suggestion ? Guest guest Posted August 13, 2005 Report Share Posted August 13, 2005 I have found that chronic issues which affect the tonsils and adenoids to make good progress with Kiiko's protocol for acupuncture and also having patient use a salt water irrigation through nose. The bathing of the irritated tissues is helped by the warm salt water and this also acts to heal tissue. Micro-organisms seem to have issue with the salt.
