Treat the pattern , but often it is qi, phlegm and or blood stag in lower abdomen. Most women can avoid surgery. I would always do some local ashi points and/or zigong xue the extra uterus point. Guizhifu ling wan or variations may be helpful Endo is more difficult. Almost always there is blood stasis. Symptoms may be managed with TCM, but total resolution I haven't seen. In the case of infertility, I have had patients with endo get pregnant, but after year or so of treatment. I would palpate for ashi points in lower abdomen and gb27, chong mai, dai mai, etc. Shao fu zhu yu tang for stasis in lower abdomen, or sparganium tabs by ITM: combine with other formulas if necessary to address the whole picture.. Randine Lewis has an interesting discussion on endo on her website fertilityretreats.com. An tai yin is a formula for restless fetus causing abdominal pain. You might try needling ear uterus and ub 32. I think " Acupuncture in Pregnancy and CHildbirth " is a good reference. I can't remember the authors name at the moment. The lady with painful intercourse may benefit from liver channel points Liv5 also is good for genital pain, although i agreee maybe try aggressive energy treatment with her since it sounds like there is deep rooted stagnation.
