Cold limbs but chest is hot " (p. 186) In other words, the Cold limbs is the false symptom. Though I want to caution readers that Cold limbs and a warm chest also can occur when there is both extreme Yang and Yin Deficiency. Also in certain scenarios where there is true Heat in the Interior and Wind Cold in the Exterior. A burning sensation in the anus also can occur when there is both real Heat and real Cold. In cases where there is both real Heat and real Cold, there will be other symptoms and signs of Cold. What Maciocia is saying is that if you run across a case where all these symptoms and signs of Heat are present but there's that one puzzling sign that doesn't fit of the limbs being cold, chances are it's a false sign and there really isn't Cold. In other words, you don't have to include something in the formula to warm those limbs. (Note for students: This doesn't mean that every single herb in the formula has to have Cool or Cold thermal energy. It means that the thermal energy of the formula overall should be Cool. Also that most of the herbs in the formula do have Cool or Cold thermal energy or at least neutral.) I'm not satisfied with the brief intro that Maciocia gives to True Heat - False Cold so hopefully some of the professionals on the list who have experience with True Heat - False Cold will flesh this bare basics intro out some more.
