Heart yin deficiency combined with Kidney yang deficiency

Hi all,

I have this patience with red tongue, crack in the

middle of the tongue all the way to the tip, no

coating, insomnia, slow pulse, constipation, body

wasting, palpitation, lower back pain, heart heat,

mouth cankers, low blood pressure.

 

All the symptoms point to both kidney yang deficiency

and heart yin deficiency. However would like your

advise on which herbs and in what combination suits

better to a person with this characteristics. I

prescribed for kidney yang:

 

 

lu rong 1 - 3 grms

rou chong rong 9 grms

hu tao ren

suo yang

wu yao 3 grms

 

For heart yin deficiency

 

Suan Zao Ren 9 grms

PAnax ginseng 4.5 - 9 grms

longan Fruit

 

Any thougths?

Hi Rodrigo,

this is an interesting question, and it is good to see

an actual case study on this list. You have provided

quite a bit of information, but there are other things

we should know before offering advice.

which symptoms does the patient complain most about?

What is the main reason s/he came to see you? Does

the patient primarily feel hot or cold? Thirsty? Are

there nightsweats? Is the patient in old age or still

young? Was the back pain due to any injury?

From your description so far, it seems like the yin xu

is much more severe than yang xu.

The only indications I can see of kidney yang xu would

be the sore back and slow pulse. Possibly the blood

pressure. But the slow pulse could come from other

sources. Is the patient athletic for instance?

From what I see of your formula so far, and from what

symptoms we have seen so far, I would think the

formula might be too warming and too strongly

tonifying, and would think about removing the ginseng

and the lu rong.

The patient's tongue seems to indicate a lot of heat,

as do many of the symptoms, so it might be most

important to deal with the heat and yin xu first,

while gently supporting the yang.

Rou Cong Rong and Hu Tao Ren seem like good choices to

deal with the constipation and to tonify kidney.

For the insomnia, heart heat/yin xu it would be

important to to clear deficiency heat as well nourish

yin/calm spirit. Suan zao ren seems appropriate choice

to help sleep, but I would pair it with Bai he rather

than Longyanrou, which is perhaps too warming. Bai zi

ren is neutal and would help unblock the bowels as

well as treat insomnia, so that could be a good choice

as well.

Huang Lian is great to clear heart heat and help mouth

sores, but since your patient has low blood pressure,

it might not be appropriate choice. It might also be

too drying. Deng xin cao could be an alternative

choice. Zhi zi would also clear heat and help

insomnia. Mai men dong to replenish fluid and help

insomnia as well as clear heat.

Anyways, these are just a few ideas. It is hard to

really say over the internet, without knowing more,

but it is fun to think about, and hopefully you will

find these ideas interesting.
