There are several differential diagnoses of hemorrhoids, so to answer the question in more detail, we would need to know more about the hemorrhoids. For instance, are they protruding with fresh red blood, or is the blood turbid? Do they protrude only when coughing, sneezing, and defecating, or are they always out or always inside? We would also need to know if the tongue is red or purple or pale, and information about the pulse to decide if the hemorrhoids are related to hot/cold or excess/deficiency. That being said, there are some common diagnoses of hemorrhoids: Dry Heat Damp Heat Qi and Blood Stagnation Qi Deficiency (Middle Qi falling down instead of holding up) The first three conditions are often (but not always) excess, while the last is deficient.
