The doctors are suggesting he take on a pretty heavy regime of ARVs now: Fuzeon/T20 (injection) Prezista 2caps Norvir 1 cap Isentress 1 cp Viread 1 cap 2x a day We have been given a dvd to learn about the preparation of the Fuzeon and need to do a short course so we can learn how to do it. I feel so defeated,I wasn-t hoping for a miracle (after 15 years we have little hope of a miracle left) but I just wanted to see a little improvement other than Frank feeling a little better physically (which hasn-t been so true the last few months anyway what with the phlebitis followed by bronchitis etc). The only good thing was that the mouth ulcers had finally disappeared but this was thanks to DIFLUCAN which in 10 days cleared them up for 3 months. I would like to ask for the doctors to do one last round of bloodwork before we decide whether to proceed with their suggested protocol just to see if the NAC, ala, selenium have kicked in yet. Since this is so involved, I am going to let Tony, Marc, and others more knowledgeable than I reply to you. However, January was a few months ago. I would ask for another round of blood work and see if there has been any improvement and go from there. Why has it taken the doctor so long to review the blood work? I would ask that it be reviewed immediately. And how much OPC is he taking? Please try not to feel defeated. Sometimes, depending on he situation, it takes a longer time to heal. Low voltage electrical current applied to the blood (over the radial and ulnar arteries) has been shown to be an effective way of ridding the blood of HIV and other viral particles. F.sseem to have beoome second class patient since stopping ARVs altho initially it was the doctors that stopped him due to the liver probs. Anyway, the doctors sent a blood test tothe US to be tested to see if his cells were suitable for the use of a new ARV drug (MAVIROC) and the hospital was meant to call us when the results arrived. They forgot, and after much naggin my husband called them and they slotted us an appointment in. Usually he has blood work done every 2 months and reviewed 10 days after the blood work is done but things have got a bit lax since he has been off the ARVs (one nurse even cancelled the CD4 and other HIV marker tests in October because she thought it wasn't worth doing them as F. refused to go back on therapy in September. Re OPC, he took 6 caps a day for 9 months and then 2 months ago dropped to 4 caps a day. The fact that your hubby's CD4 and liver markers are stable, is something that many people in similar situations would view as a positive outcome. CD4 percentage is not as important as absolute CD4 count in predicting ADIs (Aids defining illnesses). A recent research paper had the following to say about this: "..in a multivariable analysis adjusting for absolute CD4, CD4%, and other clinical and demographic variables, the absolute CD4 but not the CD4% was associated strongly with developing an ADI. The results suggest that CD4% adds little further predictive information after accounting for the absolute CD4 count for the short-term risk of developing an ADI. The absolute CD4 count is the more important measure of immune status and is preferred over the CD4% for making treatment decisions in HIV-infected adults." (http://www.medscape.com/viewarticle/487533). The same is true for the the CD4 ratio (which is the ratio of CD4 cells to CD8 cells). CD4 cells can be converted very easily into CD8 cells thereby changing the ratio dramatically. Both CD4 percentages and CD4 ratios are simply INDICATORS. They do not necessarily reflect the actual health of the person. That is why some people with low CD4 counts may be much healthier than those with high CD4 counts. I see it every day. One cannot make decisions based on the markers but should rather be guided by one's actual health. Therefore the question is: Does you hubby now feel better or worse than when he was on the ARVs some time ago? Do you observe that he is healthier or not? The ARVs are powerful anti-fungals and anti-microbials over and above the other things they do. Therefore, they may lead to improvement in the short run. However, their effects on the liver and other organs will be felt in the months to come. My suggestion is to continue with the supplements that he has been taking since January for another 2 or 3 months and monitor his actual health. If his health remains stable or improves, the markers should not guide your actions. Your husband took the ARVs for 15 years and it will take some time to undo the damage caused by them. However I am a generally positive person and since last summer his overall physical well being has improved (digestion in particular) and towards November he was feelig quite a bit better and started to jog and do some weights to build up his muscle and physical strength and I was confident that some form of improvement would come in the blood tests eventually but was okay with stable results considering his general health. However in January he got the phlebitis which set him back and lasted 2 weeks, then he got a cold that lasted ages, then the kids and him developed a really nasty cough and his turned into bronchitis and he had a temperature for 2 weeks with severe night sweats altho we managed to stay off the antibiotics. Altho he still has a cough now it is not so bad as it was and the temperature has cleared up altho the night sweats have not. He has lost some apetite again too. So altho he is still better than he was last summer in particular regarding the liver we are worried about these infections. Also we were expecting some liver markers to improve and some effect on the CD4 count. Also when I pointed out to the doctors that the natural remedies seem to having the almost the same effect as the ARVs were without the side effects and to remember F was feeling better than before too , they pointed out the the % and the ratio had kept falling and that this was an important factor not to ignore. We haven't stopped the alternative treatment nor do we plan too and we have convinced the doctors to test him once more before our decision on the ARVs. On 1 June we will go for the Fuzeon course and that day they will do his blood work one last time. They will also give me time with the doctor to ask any questions we may have (ha ha, we, they already know who will be asking the questions ...I am definitely the worst patient on tha front and I am not even the patient!!) On the basis of these results we will then decide whether to follow the doctors suggested ARV regimen or not. Obviously the pressure from them is to go the ARVs. For me a decision to start will mean defeat altho F. will continue with alternative meds too. F (who always expects the worst) was less disappointed than me and is almost resigned to going bacnk on treatment. We now how a little more than a month and plus the chance to see ifthe NAC, ALA, selenium, etc are helping too. Anyway we have a month to go so I am going to try and get F to do another liver cleanse (not easy as he was pretty disgusted by that) and some more coffee enemas, Castor Oil packs. Plus hoping and praying for some positive signs in the next lot of bloods to give me some AMMO for refusing the ARV. Thanks for your thoughts and advice. Although I must say, I don't think it has taken days. I believe a couple of posts I made were never posted. I think sometimes has a glitch.
