Hi Everybody!

 

Here is a new case. This man has so many patterns that I don't know where

to start:

 

Patient number: 310040754 Date of birth: 1-8-53

 

Presents a major complaint of a neck problem which causes him to twitch in

an involuntary effort to correct the vertebral alignment. This problem

dates to a car accident in 1975, lately has been getting worse. He has

tried physical therapy and chiropractic with partial relief.

15 years ago he had the hiccups for 2 to 3 weeks, finally got pills from

the doctor to stop them.

He has had a bump on the back (which looks like a fatty tumor to me) for 15

years. He has a cyst in the crack of his butt that comes and goes. He puts

cortisone cream on it and it goes away. Forgets to use the cream and it

comes back.

He once had a yeast infection on his penis, which he believes he got from a

woman.

He used marijuana, speed, cocaine, barbiturates etc. - everything except

heroin in the 60s and 70s. He drank heavily during the same period.

He wears glasses for near and distant correction.

He used to get wax build-up in his ears which the doctor would clean out

every 5 years, now he uses drops to prevent this.

He has shortness of breath when he is worked up.

He has Attention Deficit Disorder.

He fainted at his Dad’s wake.

He used to have hay fever 10 to 20 years ago.

He frequently gets indigestion from eating too fast.

He had black bowel movements about two months ago.

He has frequent urination when he is at home - every ½ hour, normal

intervals when he is out.

He has a trick knee - right side.

He used to suffer from fevers in California.

He frequently becomes impatient with himself.

He gained 10 to 15 lbs. last summer.

He has always had balance problems.

He has numbness or tingling in his hands, mostly at night.

His heels hurt - “plantar fasciatus” according to a doctor.

His father (deceased) had cancer, his mother (deceased) heart trouble, high

blood pressure.

He takes Advil and Motrin at night to calm his neck.

He had carpal tunnel surgery two times in 2000, July and December. Both wrists.

 

Observe:

 

Excess, rapid or jerky movement. Yes. (heat, often liver or heart)

Small movement, continuous fidgeting especially of the legs. Yes.

(deficient heat in the kidneys)

Red color (may indicate Inflammation, fever or infection) Sunburned

Small pointed head, fast moving & small hands " Fire Type " . Yes.

Fat body. Yes.

dry nasal passage or yellow phlegm, heat pattern

Pale nails. Yes. Blood deficiency.

Tongue color. Red with redder tip. Full heat, Heart fire.

Tongue shape. Thin pointed. Yin deficient.

Tongue coating. White at the center, mottled. Pathological factor.

Tongue moisture. Wet. Yang qi deficiency.

 

Inquire:

 

Gradual onset of headache. Yes. Deficiency.

Location of the headache. Anywhere, over eyes, temple a lot of times.

Character of the headache pain: all kinds, mostly dull pain. Deficiency.

Condition of the patient while having the headache. Incapacitated.

Sudden onset of dizziness. Yes. Excess.

Pain that is diminished by heat. Yes. (cold)

Pain that is diminished by cold. Yes. (heat)

Pain that is relieved by touch or pressure. Yes. (deficient) if someone

else massages.

Pain with bloating or a sense of fullness. Yes. (excess)

Sharp stabbing pain usually in a fixed location. Yes. (blood stasis) before bm.

Sensation of heaviness. Yes or no. (damp cold, damp heat, qi deficiency or

bi syndrome)

Dark Yellow or Reddish Urine. Sometimes.

Scanty urination. Sometimes.

Urgent diarrhea, especially if yellowish with burning anus. Yes, if eat

spicy food.

Undigested food in the stool. Yes. Peanuts.

Not able to fall asleep. Yes. Sometimes.

Dream disturbed sleep. Yes. Sometimes.

Wake up early and not able to fall asleep again. Yes.

Change in sinews, such as weak or stiff. Yes. Hands.

 

Listen & smell:

 

Weak sounds from the abdomen. Yes. (empty or deficient pattern)

 

Palpate:

 

Systolic blood pressure (Optimal <120; 104

Diastolic blood pressure (Optimal <80; 80

Heart rate. 60

Slow pulse less than 70 beats per minute. Yes. (cold or insufficient qi)

Hidden pulse is an extreme form of the sinking pulse. Yes. (Deficient Yang

if weak.)
