I will share western diagnosis as well as TCM I have a client who recently miscarried and had a subsequent D & C. As a result of this D & C she has been diagnosed with Ashermans syndrome which is uterine scarring that form on the uterine wall. In severe cases - the uterine wall adhers. The primary sx becomes reduction or lack of menstruation. From a TCM perspective - she presents with symptoms of blood deficiency - including very light menstraul cycle with pale blood, fatigue, poor sleep. She has no pain in the uterus and no cramping or other stasis symptoms. Her bowel movements lean toward loose. Her Pulses are very minute and think, her tounge is scalloped, puffy, pale and quivery. I recognize that, although she is deficient, the asymptomatic stasis in her lower abdomen needs to be addressed without depleting her further. I do not create my own formulas and work with herbs periodically. I would like to help her the best I can. I have patients that have been diagnosed with this or that WM syndrome and although I'm not suggesting one should ignore pt's primary care provider's diagnosis, I'm more inclined to follow my assessement of the patient as they present themselves to me during our session. If your patient's signs and symptoms are of blood xu, then you should treat that, blood xu. Based on your post, I do not see any signs of stasis (remember, I'm not taking the WM diagnosis under consideration at this time). Bear in mind that qi vacuity can lead to blood vacuity and to stasis and it is very possible that the mere act of supplementing and regulating qi could lead to a removal of any stasis secondary to qi vacuity. You may want to think in lines of Ba Zhen Tang with modifications to fit your pt's pattern. Keep in mind the loose stool, the absence of pain, the color of the menses the puffy, pale tongue, fatigue, etc., when adding or subtracting ingredients. A few blood/qi regulating herbs would not hurt if carefully chosen with appropriate dosage.
