Guest guest
All, Does anyone have a copy of a consent form they use with clients? If possible, would I be able to get a copy? I would like to institute this for the new year. Judy Di Canio http://www.frequencybalancing.com
Guest guest
I'm going to enable my FTP priviledges at my website, but it won't be enabled until midnight tonight. So, tomorrow I can give you a URL to click on and it will automatically download the informed consent/arbitration forms I use. They came from one of my malpractice carriers so you know it has to be rock solid and air tight legal-ease which ironically rhymes with dis-ease. : ) -al. jdicanio wrote: > > All, > Does anyone have a copy of a consent form they use with clients? Guest guest
Oops, it downloaded as gibberish. Try this URL instead: ftp://gancao.net/consent.arbitration.txt Al Stone wrote: > > Okay, just click here and it'll automatically download. > > ftp://gancao.net/consent.arbitration.doc > > -al. > > jdicanio wrote: > > > > All, > > Does anyone have a copy of a consent form they use with clients?
