Lunch usually left overs or a salad with chicken, so that's hard to say. Eats a lot of stuff, soups, eggs, whatever is here. Been craving the carbs. Dinner salad and some kind of fish. Every once in a while pasta, but meat and salad mainly. Mexican a lot. Snacks and Vices: A glass of wine now and then but has been bothering sinuses lately so has cut back. Not much snacking, cheese maybe, dried fruit, ice cream Symptoms worse at night Tongue (her description, she is a friend who lives out of state): Medium pink, doesn't appear to be coated, maybe a little white all over, no red edges or spots, no cracks, no purple, no swelling, moist Body weight : good muscle tone, overweight Strength : moderate Skin : dry, soft, cool, olive complexion Hair : coarse, greying, thick, wavy, abundant Teeth : moderate size, yellowish Gums : pink, thick, strong Eyes : dry, itchy Hands and feet : moderate size, large, thick Voice : loud, deep Immunity : good, strong Appetite : steady Thirst : variable Stool : dry, hard, some foul odor, well formed Movement : one a day Urine : pale Physical activity : moderate Endurance: moderate Sleep : sound, moderate, 8 hrs. Sensitivities : dislike of damp & humidity Emotions : fearful & anxious at times Preferences : prefer warm food and drink, dislike of greasy foods DISORDERS: night sweats, foggy headed, spacy (sometimes), bad breath, tinnitus, blurred vision, night blindness - floaters, low back pain, pale, withered or brittle nails, varicose or spider veins, puffy eyes in the morning, lumps/cysts, mood swings, frustration, resentment, stuffed nose, chronic allergies (possible), absent menstruation, hot flashes, breast lumps Diagnosis: Stagnant Liver Qi with Liver Blood Deficiency Treatment Principle: Disperse the Liver and regulate Qi, tonify Liver Blood Treatment : Xiao Yao San Stagnant Liver Qi : mood swings, frustration, resentment, suppression of menses, tinnitus Liver Blood Deficiency: dry eyes, pale, withered, thin or brittle nails, blurred vision, night blindness-floaters, absent menstruation, mental irritability (resentment, frustration, mood swings), dry skin, tinnitus Other possibilities: Arrogant Liver Yang - Liver Yang Rising (combined Excess and Deficiency): tinnitus, irritability (mood swings), hot sensations in the body (hot flashes), burning sensation in face (she said her face feels like it must get red ... but she never really looked), misty vision (blurred) This would help to confirm the above: Excess as the Stagnation, Deficiency as the Deficient Liver Blood Damp Heat Invading Spleen: loose stool w/offensive odor, dislike of greasy foods Food Stagnation: bad breath, stools w/offensive odor * These could be addressed additionally with diet modification (warming, drying foods, no raw, no dairy, mild carminative spice but no mexican hot stuff, low salt, low fat). Liver Yin Deficiency: night sweats, tinnitus, scanty or absent menstruation, muddled vision - blurred, dry eyes * All (except the night sweats) can be symptoms of Stagnant Qi and Deficient Liver Blood Kidney Yin Deficiency: night sweats, afternoon hot flashes, tinnitus, low back pain, blurred vision * Other than low back pain and night sweats, these can all be signs of the diagnosis. She does not display any other signs that would confirm Kidney Yin Deficiency such as: hot sensations palms, soles, chest, dry mouth at night, constipation, dark, scanty urine, nocturnal emissions, emaciated appearance, fatigue or general weakness, nervousness and hyperactivity etc. Her tongue is not red or dry and her pulse is not fast (60 is slow for her age). Her constitution seems strong. Liver Blood Stagnation: Varicosity's, varicose veins, cysts, breast lumps * She doesn't have sharp stabbing pain nor does she have a purplish tongue. Therefore I suspect long standing Qi Stagnation as cause. I will be happy to address any questions regarding the above and I am open to enlightenment if I am incorrect with this! Her constitution seems strong. Not every symptom and sign needs to be present for there to be Yin Deficiency. The more severe and the longer any imbalance has gone on, the more symptoms and signs will be present. The more severe the imbalance is compared to other imbalances, the more its symptoms and signs will manifest in comparison to those of less severe imbalances. Tongue body color often is an average. The pale tongue of Blood Deficiency often will trump other conditions, or the tongue color may average out to pinkish in cases where there is Blood Deficiency and Heat. There can be a normal colored tongue with some cases of Yin Deficiency. In some of the entries in the Materia Medica you will see the phase " Yin Deficiency with Heat " . This means that the Heat symptoms are particularly marked and bothersome. In these cases the tongue would be red. In cases where the Heat is not so severe, the tongue may be normal-colored. There is always some (Deficiency) Heat present in Yin Deficiency, but how much will vary. (Note for those new to TCM: Yin cools and calms the body. Deficiency Heat is Heat that is present because there is not enough Yin to cool the body properly. Excess Heat is that which is caused by the person eating too many foods with warming energy, taking too many herbs with warming energy, infection, sun stroke, Qi Stagnation which has turned into Heat,etc. Yin also moistens. Yang heats, activates, and dries. Excess Yang can be the cause of Dryness, but Blood Deficiency also can cause Dryness. When considering any problem, consider if the cause is Deficiency (not enough of something which is needed) or Excess (too much of something).)
