There are signs which you will be noting. What color is her tongue and the tongue coating? Pale tongue tissue and white coating point to Cold; red tongue and yellow coating point to Heat. Pale face probably is Cold; red face probably is Heat. Red blotches probably is Yin Deficiency. Hard, tense muscles probably is Cold. (Cold contracts.) (These are general statements and there are exceptions, but this should give you some ideas when the patient can't answer questions.) You're going to have to have some idea if it's Cold or Heat because the treatments for migraine can vary so much. >That's why I wondered if there were some universal acute migraine >headache pressure points I might use for a few minutes...before reaching >for the injectable Demerol and Phenergan. If s/he's like me, the Demerol and Phenergan aren't going to do that much good. But they could blunt the pain and nausea enough for her to answer some questions and work with you. As for general points for headaches, check the headache chapter in Acupressure's Potent Points: Gall Bladder 20 (Gates of Consciousness - this one gets used for a lot of things), Governing Vessel 16 (Wind Mansion), Bladder 2 (Drilling Bamboo - if there's any sinus and nasal congestion present, this one can work wonders - there often will be sinus and nasal congestion with a migraine if Wind Cold is the Root), Governing Vessel 24.5 (Third Eye Point), Stomach 3 (Facial Beauty), Large Intestine 4 (Joining the Valley - this one gets used for a lot of things - contraindicated in pregnant women - constipation often is a factor in headaches but this is not the only reason this point is so good for headaches - it tonifies Qi ( when combined with St 36 and other points - good if Qi Deficiency is Root of headache - head will feel like it is caving in) - it dispels Exterior Wind (good if Wind Cold is Root of headache) - it harmonizes ascending and descending of Qi (good if migraine is due to Liver Yang rising) - it stops pain - it removes obstructions from the channel), Liver 3 (Bigger Rushing), and Gall Bladder 41 (Above Tears). What helps me the most in the TaiYang stage is pressing or massaging Small Intestine 3 and a large heating pad over the shoulder blades (a number of Bladder points) and a second heating pad over the Sea of Vitality area (Bladder 23 and Bladder 47). The general points I gave you don't do anything for the migraines I have. As you get more familiar with points and start working with them, you'll figure out a lot of things on your own. You'll start automatically having some idea of what points will work for which individual. Practice on yourself the next time you have a headache. Look at the color of your tongue and tongue coating. Notice if there is any pain, cold or heat sensations, numbness, tingling, etc. along the course of a meridian. Are the muscles stiff or relaxed in the back, shoulders, and neck? Instead of reaching for an aspirin, start to analyze from a TCM standpoint. Do you feel hot or cold or both? Are you sweating? Etc. Taking a history like you suggested sounds good for when I might be working a patient up. But when a woman is lying down in my office in a darkened room, throwing up, holding her head, and crying from pain --- she doesn't want to answer history questions. She wants pain relief ASAP! That's why I wondered if there were some universal acute migraine headache pressure points I might use for a few minutes...before reaching for the injectable Demerol and Phenergan.
