I have written the following to give to our doctor Tuesday when they both go for their college entrance physicals. Does anyone have any experience with this? Will this letter suffice? Malinda July 5, 2004 RE: Religious objection to further vaccination To Whom It May Concern: Pursuant to the Indiana State code on Immunization Requirements, please consider this my written request to be exempt from any required or requested vaccinations based on my religious beliefs. Code follows. IC 20-12-71-14 Religious objections Sec. 14. (a) Except as otherwise provided, a student may not be required to undergo any testing, examination, immunization, or treatment required under this chapter when the student objects on religious grounds. (b) A religious objection does not exempt a student from any testing, examination, immunization, or treatment required under this chapter unless the exemption is made in writing, signed by the student, and delivered to the individual who might order a test, an examination, an immunization, or a treatment absent the religious objection.
