Guest guest
Dear Marilette: Thank you so much for your prompt responses and the hope you give us when we are struggling to find the right answers. I would appreciate very much if you could you please send me the protocol for Temporal Arteritis. I did read message # 3018 for Vasculitis, but it does not address the specific problem of the temporal arteries. The patient is 57 years old, male, and the symptoms are: 1. Low to medium level constant headache near the right temple, which has been going on for the last several months. 2. Once in a while, blurring sensation in the left eye. With deep gratitude for your tremendous help, Sincerely Suchitra =================================== Dear Suchitra, Atma namaste. Thank you for your email. Medical Background: Temporal arteritis (TA) is a chronic inflammatory disease of large blood vessels, particularly the carotid artery and its branches, occurring primarily in older adults. Simultaneous polymyalgia rheumatica is common. Focal symptoms and signs may include headaches, visual disturbances, temporal artery tenderness, and pain in the jaw muscles while chewing. Fever, weight loss, malaise, and fatigue are also common. ESR is typically very high. Diagnosis is most often by biopsy. Treatment with high-dose corticosteroids is usually highly effective and prevents visual loss. Etiology and pathogenesis of temporal arteritis (TA) are unknown. It is rare in patients over 55 and is slightly more common in women. The prevalence is highest in people of northern European origin and can reach 15 to 25 cases/100,000 population. Pathology Vasculitis may be localized, multifocal, or widespread, most often involving temporal, cranial, or other carotid system arteries. Segments of the aorta, its branches, the coronary arteries, and the peripheral arteries may also rarely be affected. The disease tends to affect arteries containing elastic tissue, with granulomatous inflammation primarily of the media; lymphocytes, epithelioid cells, and often giant cells predominate. The elastica is disrupted and the intimal layer is markedly thickened, with narrowing and occlusion of the lumen that can produce ischemia. Symptoms and Signs Presentations are diverse, depending on the distribution of the arteritis, but typically include severe, sometimes throbbing headache (especially temporal and occipital), visual disturbances (amaurosis fugax, diplopia, scotomata, ptosis, blurred vision), and pain in the masseter and temporalis muscles on chewing (jaw claudication). Patients may report scalp pain with combing their hair. Systemic symptoms may include fever, fatigue, malaise, unexplained weight loss, and sweats. Patients may also present with symptoms of polymyalgia rheumatica (PMR), carpal tunnel syndrome, radiculopathy, and, rarely, pulse deficits similar to those in Takayasu's arteritis. Blindness caused by ischemic optic neuropathy occurs in & #8804; 10% of patients but can occur suddenly and irreversibly. Physical examination may reveal swelling and tenderness with nodularity over the temporal arteries and, rarely, bruits over the large vessels. Diagnosis TA should be suspected in patients > 55 with almost any new symptoms or signs compatible with ischemia of an artery above the neck or with jaw pain on chewing, new-onset headaches, or tender temporal arteries. Symptoms of PMR increase suspicion.... TA can be diagnosed clinically on a provisional basis, but the diagnosis should be verified by temporal artery biopsy.
