Guest guest
Dear Sue, Namaste. Thank you for your email. Medical Background: " Phimosis " is a vague term. In common usage, it usually means any condition in which the foreskin of the penis cannot be retracted. Most infants are born with a foreskin that does not retract. This is normal. The specific medical definition of phimosis: True phimosis is tight non-retractable foreskin caused by Balanitis Xerotica Obliterans (BXO) and is distingished by a whitish ring of hardened sclerotic skin at the tip of the prepuce. Histologic examination by a pathologist is necessary to confirm the diagnosis. If BXO is not present, then true phimosis is not present. Balanitis Xerotica Obliterans was first described by Stühmer in 1928 in Germany.Balanitis Xerotica Obliterans (BXO) and Lichen Sclerosus et Atrophicus (LSA) are two names for the same disease. BXO/LSA is a skin disease of unknown etiology. It occurs in both males and females. LSA is the name applied when the disease appears in a female or a male in other than the genital organs. BXO is the name traditionally used when the disease afflicts the male sexual organs. An older name is kraurosis glandii et praeputii penis. BXO may be briefly defined as a chronic, progressive, atrophic, sclerosing process of the glans and prepuce leading to urethral stenosis. Operations on the genitals, such as circumcision and dorsal incision, are not etiologic factors in our opinion. In contradistinction to Stühmer's2 belief that the disease occurs only in young adults, it may occur at any age. The onset of BXO is as a rule insidious, although it may appear as an acute disease with pain, itching burning, prickling and profuse purulent secretion. It becomes chronic and in most cases slowly progressive up to a certain point, where it remains stationery. In patients who have not been circumcised there is almost constant involvement of the prepuce in the form of a sclerotic, constricting band, which is located 1 or 2 cm. from the distal end. The involvement of the glans may be diffuse or as mottled patches. The admixture of the white plaques and normal red tissue of the glans may give rise to a mosaic appearance, and if telangiectasia is present there may be a resemblance to roentgen ray dermatitis. Fissure and erosions are rather common, and hemorrhagic or serous bullae often accompany the cicatricial changes. There are reports indicating that the condition may gradually develop into carcinoma. One of the noteworthy features of balanitis xerotica obliterans is the gradual narrowing of the urethral meatus (foreskin), which often leads to almost complete stricture.
