Dear Carolyn,

Greetings.

MEDICAL INFORMATION: NARCOLEPSY

" Narcolepsy is an uncommon sleep disorder. It is marked by recurring

irrepressible attacks of sleep during normal waking hours, as well as by

cataplexy, sleep paralysis and hallucinations.

The cause of narcolepsy isn't known, but the disorder tends to run in

families, suggesting a genetic predisposition. While narcoplepsy has no

serious medical consequencies, it can be frightening and may increase the

risk of accidents.

It usually begin in healthy adolescents or young adults and persist

throughout life. A person with narcolepsy is overcome by sudden attacks of

irrepressible sleepiness that can occur at any time. Falling asleep can be

persisted only temporarily, but once asleep, the person usually can be

easily aroused. A person may have many attacks in a single day or only a

few, each usually lasting an hour or less. The attacks are most likely to

occur in monotonous situations such as boring meetings or long periods of

highway driving, The person may feel refreshed on awakening but may fall

asleep again several minutes later.

A person with narcoplepsy may become paralyzed momentarily without losing

consciousness (a condition called cataplexy) in response to a sudden

emotional reactions such as anger, fear, joy, laughter, or surprise. The

person may become limp, may drop something being held, or may fall to the

ground. The person may also have occasional episodes of sleep paralysis;

when just falling asleep, or immediately on awakening, the person

experiences a sense of wanting to move but is unable to do so. Vivid

hallucinations, during which the person sees or hears things that aren't

there, may occur at the beginning of sleep or, less often, on

awakening. The hallucinations are similar to those of normal dreaming but

more intense. Only about 10% of the people with narcolepsy have all these

symptoms; most have only a few.

Diagnosis.

While the diagnosis is usually based on the symptoms, similar symptoms

don't necessarily mean that a person has narcolepsy. Cataplexy, sleep

paralysis, and hallucinations are all common in young children and occur

occasionally in otherwise healthy adults. If a doctor is uncertain of the

diagnosis, the person may be sent to a sleep study laboratory. "
