Attorney or Authorization to

Release Information before you are admitted to a hospital and the nurses and doctors

won’t tell you or the person you’ve signed for an Authorization to Release

Medical Information about your case or give you any record copies of your care,

Dear Everyone,

 

This post from Dr. Lance is extremely important in

light of the April 14, 2003 HIPAA Privacy Law. The

hospitals will not release any information to anyone

if this form is not filled out. I would fill a

release form out for everyone that you want to have

access to your medical records or information about

your condition.

 

Contact your HMO or the hospital where you receive

care and make sure you have everything in order so

that you will not be left cut off from those who care

about you if you are hospitalized.

 

 

While thousands in the HMO can snoop and paw through

your private electronic medical record at will, the

hospital will make a huge issue about " protecting your

privacy " from your family or other loved ones if you

are ever incapacitated. It could spell your death.

 

If you are in poor health, make sure you have someone

lined up who can access your medical information and

be your advocate if you end up in the hospital.

 

Do you need to sign a release so that all of your

friends can know how you are doing? Probably--so look
