Updated Questionnaire for Vision Improvement Materials

Hi Folks,

 

I am constantly updating my questionnaire as is necessary. I am

getting alot of requests lately and I'm about a month behind in

sending out materials. My friends and family are advising me to

do this professionally so I may be giving lectures and or

seminars on vision training in the future for $$$. No definite

plans are in the works.

 

Anyone that requests materials before I start doing it for money

will get them for free, as promised.

QUESTIONNAIRE: NEWEST VERSION: 1/24/03

 

 

• (2), (3), & (4) are things you ABSOLUTELY NEED to be able to

use the materials.

 

• You " ll also need a printer.

(1) Your make of computer, Mac or PC

 

(2) If you have MS Word or other word-processing program

 

(3) If you have Quicktime or Photoshop or other graphics-viewing

programs. You can download a $30 shareware program at

(4) If you have Adobe Acrobat Reader. If you don't you can

download it for free at http://www.adobe.com For best results get

Acrobat Reader 5

 

(5) Your age, vision problems, and vision history, ie do you wear

glasses, details of your prescription, what is your diagnosis, etc.

If you wear glasses please be as explicit as you can about the

kind of prescription you have and your " numbers. "

 

(6) Have you ever had an accident caused apparently by your

poor vision

 

(7) Have you ever suffered an eye injury

 

(8) Do you drive a car, do you need glasses to drive or have you

noticed recent difficulty seeing in the distance?

 

(9) If you have ever done anything w/re to vision training or

natural vision improvement before, what kind of NVI it was, and

what you perceive to have been the results.

 

(10) Any info regarding any diagnosed eye diseases you have

and/or any other general health problems

 

(11) If you exercise (sedentary to strenuous exercise & how

often) and your general level of fitness

 

(12) Your diet (as much info as you think is relevant. If you

commonly have chocolate cream pie instead of dinner, this is

relevant.) I don't consider eating meat necessarily a " poor diet. "

Please be as descriptive as you can on this one.

 

(13) Do you take any vitamins or other nutritional supplements

 

(12) List any and all medications that you use, including over-the

counter meds

 

(14) If you are a smoker or anything more than a social drinker

(one glass of wine twice weekly = a social drinker)

 

(15) The nature of your work (computer professional, etc.) and do

you use computers when not at work as well.

• I gave lectures on Vision Training while working in India but

have never done so here in the USA. At some point I may do this

here professionally. For the meantime I am passing on all the

information I have with no charge. Please EMAIL ME PRIVATELY

whenever you have questions about orthoptic exercises. I don't

necessarily check the boards all that often. You can read my

• Please s directly below for some suggestions

re: " computer ergonomics. " This is very, very important. Nearly

everyone's computer habits could be improved. Computers are

here to stay but they are a major source of eyestrain, and their

improper use contributes to the worsening of all eye problems.

• If you have myopia ONLY (distance vision problem) and no

other conditions, you might want to try PLUS LENS THERAPY

instead of the orthoptic training that I studied. You can find out
