Tell your health-care practitioner and pharmacist if you are taking any other prescription or over-the-counter drugs, as well as any vitamins, herbs, or other supplements. Possible drug interactions include the following:

 

Taking other blood pressure medications can increase the blood pressure lowering effects of lisinopril. It is best to stop taking all other antihypertensive drugs (especially diuretics) 1 week before beginning treatment with lisinopril. Otherwise, if you are already taking a diuretic, you may experience a rapid and dangerous drop in blood pressure when you begin taking this drug. After your health-care practitioner sees how you react to lisinopril, other high blood pressure medications can be restarted as needed.

Lisinopril increases blood levels of potassium, particularly when taken with potassium-sparing diuretics. Too much potassium can cause heart rhythm disturbances.

Combining lisinopril with loop diuretics increases the risk of postural hypotension (low blood pressure and dizziness when you stand up).

Do not combine lisinopril with potassium medications, such as K-Lyte or Slow-K.

Concomitant use of lisinopril with anti-diabetic medications (e.g., insulin, glyburide) may enhance the glucose-lowering effects of these medications. Careful monitoring should be conducted when combining these medications.

Do not combine lisinopril with interferons. This combination is associated with an increased risk of blood problems.

Combining lisinopril with allopurinal increases the risk of serious skin reactions.

Taking lisinopril with lithium can result in toxic levels of lithium in the blood.

Phenothiazine tranquilizers and antiemetics may increase the effects of lisinopril.

Aspirin and other NSAIDs (such as ibuprofen and indomethacin) can reduce the blood pressure lowering effects of lisinopril.

Separate doses of antacids and lisinopril by 2 hours.

Lisinopril increases blood levels of digoxin.

Alcohol may further lower blood pressure. Use with caution.

Consult your health-care practitioner before taking over-the-counter cough, cold, sinus, allergy, or weight loss medications. Many of these can raise blood pressure.

Food, Vitamin, and Herbal Interactions

Do not take herbs that increase blood pressure, such as ephedra (ma huang), ginseng, goldenseal, licorice, and saw palmetto. Consult your health-care practitioner before taking substances that may lower blood pressure, such as calcium, soy, or garlic. To minimize possible interactions, take lisinopril and iron supplements 2 or 3 hours apart. Because ACE inhibitors such as lisinopril raise potassium levels in the body, do not take potassium supplements or use salt substitutes containing potassium, and ask your health-care practitioner how many potassium-rich foods (such as bananas, prunes, raisins, melons, tomatoes, citrus fruit, and orange juice) you can safely include in your diet. Also talk to your health-care practitioner about salt intake.
