It depends on the current, the technique of action, the patient, mechanism of pain etc… In the middle of last century in St. Petersburg Laboratory of Brain they found that frequency of 77 Hz was the most effective to stimulate endorphins production. From acupuncture point of view I think it is more important to define TCM diagnosis precisely and choose necessary location (zones or acupoints) of action. It seems to me that frequency can play the second or third role in this game. Differential diagnostics of the pain syndromes … As already mentioned earlier, each system of Channels and Collaterals has its own structure, pathologies and requires specific treatment methods. To carry out an efficient therapy one has to determine the pathology location, the type of syndrome (Excess or Deficiency), pathogenesis and type of affected energy. … Below the differential diagnostics and certain features of therapy are given for the pain syndrome, depending on pathogenesis and type of effected Qi, as well as different methods of indefinite pains treatment among women and men. Differentiation of the pain syndrome according to Localization A) Tendino-Muscular Channels: Excess Syndrome: Exogenous Pathogenic Factor is the cause of disease; Pains are superficial, intensified by pressure, Local hyperesthesia, hyperthermia and edema are present; Internal organs are not affected Deficiency Syndrome The cause is usually the Exogenous Factor, though the syndrome may be also due to Regular Channel or Organ Deficiency; Pains are superficial, reduced by pressure; Local hyperesthesia, hyperthermia and reduces turgor are present in the pain syndrome area; Dysfunction of interior organs also is possible. B) Luo- Channels or Collaterals: Excess Syndrome: Pain is felt inside and may have any extension; Pain is increased in motion which «shortens» the Luo- Channel, e.g. if pain aggravates while bending to the painful side it relates with Gallbladder Collateral; Symptoms related with dysfunction of organs connected with pathological Luo- Channel may also be present. Deficiency Syndrome: Pain is felt inside and may have any extension; Pain is decreased in motion which «shortens» the Luo- Channel, e.g. if pain releases while bending to the painful side it relates with Gallbladder Collateral; Symptoms related with dysfunction of organs connected with pathological Luo- Channel may also be present. C) Regular Channels: Most often the pain syndrome is formed due to Interior Imbalance in the Yin-Yang related pair (except of the Bladder and Small Intestine, where pains are usually caused by Exogenous Factor); Pain may aggravate or wane depending on 2 hour cycle of the Regular Channels activity; To test the Excess or Deficiency of Regular Channels, one may analyze the dynamics of the pain depending on the Channel or Organ function, e.g. «inhale-exhale» for Lung or food intake for Stomach, etc.; Pain syndrome may be accompanied by dysfunction of the corresponding Organ; Additionally, pulse diagnostics is used, either its classical or simplified method. If under palpation of pulse corresponding to the exact Regular Channel the pain is intensified, this means the presence of Excess syndrome, otherwise, if the pain is reduced, this indicates the Deficiency syndrome. D) Divergent Channels: Pain syndrome related with a Divergent Channel is usually one-sided and is characterized by missed beats at regular intervals («intermittent» pulse), Also, if Luo- Channel Deficiency syndrome is present in one part of the body, one may suggest that the opposite-part Divergent Channel is affected. E) Miraculous (Extra-) Channels: Pain syndromes of affected Extra-Channels are usually caused by Exogenous Pathogenic Factors and have Excess character. Since the Extra-Channels connect different structures of the point-channel system and their pulse diagnostics is difficult, therefore the clinical manifestations must be carefully analyzed and length- variation tests of the affected Extra-Channel be used. For example, lumbar pains resulted from lifting of heavy loads and accompanied by a difficulty of back-bending are probably related with the affection of Chong Mai channel. First treatment procedures would require dispel of the Confluent point Sp4 (Gong Sun), as well as the likely Excess area painful when palpated, i.e. the extraordinary point Shi Qi Zhui (below the spinous process of the fifth lumbar vertebra, between L5 and S1). (2) Differentiation of the pain syndrome according to the affected Qi (Energy) Before any of the treatment methods are applies, one must carry out a careful differential diagnostics of the disease origin and type of affected energy: Wei Qi, Ying Qi or Yuan Qi. This will not only increase the treatment efficiency, but also reduce the risk of possible complications. Wei Qi (defensive Qi) circulated most superficially and prevents the penetration of Exogenous Factors into the body depth. Wei Qi starts to grow in the morning becomes maximum in midday, falls towards night and is minimal at midnight. So pain syndrome caused by the Exogenous Factors can release in the morning, disappear in midday, appear towards night and aggravate till maximum at midnight. Either EHF-puncture, acupuncture, or metal applications, massage and cupping may be used for treatment. Chiropractic is contraindicated because Wei Qi in the blocked areas prevents the penetration of Exogenous Factor into deeper pats of point-channel system. Ying Qi (nutrient Qi) circulates in Regular Channels, making 50 cycles per day. Each Channel and Organ have 2 hours of maximal and then minimal activity. Therefore the diseases caused by Ying Qi affection may have 2 hour intervals of pain aggravation and relief. EHF-puncture, acupuncture, point massage or chiropractic are applicable. Yuan Qi (hereditary or primary Qi) is stored in Kidney and is never in Excees. This Qi is permanently consumed during the life. Deep circulation of Yuan Qi between the Seas, Extra Channels and other structures of the point-channel system is closely related with Lunar phases. Diseases caused by Yuan Qi disturbances bear a progressive character with the patient's age, while the pain syndromes may be related with Lunar phases and menstrual cycle in women. The treatment uses EHF-puncture, deep needling, moxibustion and chiropractic. (3) Differentiation of the pain syndrome according to the Sex identity When the indefinite pain syndrome is present, having the Excess character, the following treatment may be indicated: For men to use the Yang Jiao Mai and acupoints B59 (Fu Yang), B61 (Pu Shen), B62 (Shen Mai) and B67 (Zhi Yin). Exogenous Factor is dispelled from Yang Jiao Mai through sedation of the points B59 , B61 , B62 while the Bladder Regular Channel is reinforced through the tonic point B67. Thus the nutrient (Ying) and Defensive (Wei) energies are supplied to Regular Channel preventing the disease relapse. For women to use the Yin Jiao Mai and acupoints K8 (Jiao Xin) and K7 (Fu Liu). Initial by the Exogenous Factor is dispelled from the Yin Jiao Mai by sedation of K8 point, and therefore the Kidney Regular Channel is reinforced through the tonic point K7 [AChamfrault et al. 1969; N.V.Nghi, 1974].
