I do not have experience treating this condition with AP, so I am turning to the lists for guidance. Patient has longitudinal tear in lining of anal canal, probably caused by traumatic laceration from hard stool. Specialists have tried everything from stool softeners to sitz baths to topical nitroglycerin, but fissure only healed about 50%. Now they are contemplating surgery to interfere with the cycle of internal anal sphincter spasm (in theory, cause of perpetuating condition). Surgery will involve internal anal sphincterotomy. Patients wants to avoid this surgery if at all possible. He heard that AP can accelate wound healing and relieve muscle spasm. But I do not know if it can heal anal fissure or relieve spasm of internal anal sphincter. I am considering " surrounding the dragon " with EA between GV01 and CV01, with distal points on UB channel, which Nei Jing says " enters the anus. " On internet, I find reference to one study, but do not have details or abstract (Yang WB, Kong FW. AP for anal fissure. AJA 1987; 15:384.)
