Also more information on the person and a diagnosis by pattern would be helpful. Anal fissures are pretty common but sometimes a bit recalcitrant to treatment. Any condition that causes dry stools, and the straining that accompanies chronic constipation, can lead to an anal fissure. Treatment is based upon maintaining a soft stool so as to avoid repeatedly opening the fissure and keeping the area clean (ie: washing the anus with soap and water after defecation. So various patterns that cause constipation w/dry stool are the most likely to contribute to anal fissure (my understanding is to try TX for 4-6 weeks; if no apprciable result after that time then the surgical option should be considered...) It is very likely that there is an assault from the HT, where a yin xu may have caused the SI-UB pair to run excess. SI-UB also run in tandem with DU, which in turn has a connect to the yang wei. In that sense the anal area is intimately ruled by the DU and the adjoining near channel and particularly the point UB 35. Thus fissure can be reversed by allaying the SI-UB, which can be done by balancing HT-SI. A 4-needle prescription, if all the above bears out, is: 1. One or the other UB 62 as a Master point. Palpate para-coccygial area, and see if tenderness is reduced by massaging UB 62, Once you get the right spot, find the right angle, and insert a 38 gauge 1 " needle to the depth of .2 or .3 feng. No further stimulation is necessary. 2. Find the " open " horary Wood point on channel running at the time of treatment. Use similar thin needle, and place in tendermost spot in line with the run of the channel. Stimulate twice in 20 mts for 30 secs. 3. Find similar Metal yang point on open channel. Reduce twice in 20 mts for 30 secs. 4. The 4th point could be: a. LI 20, if there is vascular injection and redness. b. LI 4 if there is tenderness. c. ST 25 if there is tenderness and tightness. The fissures have tended to be where there is some Spleen Yang Xu. If there was also phlegm and heat, then I would have to weigh up the situation very carefully, but I wouldn't categorically say that phlegm/heat would stop me doing the tx.
