That's because you only take patients who want to become a non-smoker. Before making the appointment do your best to talk to the patient and directly ask them why they are choosing to become a non-smoker? Usually they will say for health reasons. There is the world of difference between saying or thinking “I want to give up smoking” and “I think I should give up smoking”. “My office doesn’t allow us to smoke. My mate doesn’t want me to smoke”. Keep probing with open-ended questions until you are sure they desire to stop smoking for no one else but themselves. Any other reason and you are wasting your time and theirs and risking your reputation and that of homeopathy and the other modalities. You may make money today but it won’t bring you a lot of referrals. If they say they want to stop because their mate, parent, girlfriend or anyone else wants them to, DONT take them as a patient. It is essential that the patient realizes that what he or she is aiming for is to become a non-smoker Tell them: There is no point in telling yourself that you will “just have two cigarettes a day” or “only smoke when having a cup of coffee or after a meal”. If that is in their mind, forget it because it just will not work. Those two cigarettes will soon become three… or five… or ten… and before long you will be back where you started. Tell them you don’t want to waste their time or money and to wait until they are ready to quit. WHEN YOU HAVE DECIDED YOU ARE READY FOR YOURSELF TO BECOME A NON SMOKER, I HAVE AN INCREDIBLE PROGRAM THAT WILL ASSIST YOU INTO PERMANENTLY BECOMING A NON-SMOKER IF THAT IS YOUR CHOICE. Give them the suggestion and assure them that your method of detox can be simple and comfortable, extremely effective and long lasting. Permanent if they choose. Outline your treatment program - how many visits, how often, cost, etc. There are three aspects that you as a physician must deal with when people are addicted to cigarettes. Desire, Habit and the addiction. The multi-therapeutic approach is very important. Combining homeopathy, acupuncture and deep breathing exercises, and a nutritional protocol can remove the toxins and the physical cravings. The psychological aspects must also be dealt with, with behavior modification, affirmations, hypnosis/creative visualization and follow up contact. Desire is not enough to quit for many people. Every time a person has put out a cigarette they have become a non-smoker because they are not smoking at that moment. For complete success the whole person and their individualized ideas of what smoking means for them needs to be addressed. That is why choosing the correct homeopathic remedy to impregnate a customized remedy for patient is so important. It will help with both the physical and emotional issues along with resolving the addiction. It is the physician’s role to help them understand their triggers and motivations for smoking and guide them into alternatives to modify that behavior satisfactorily. Actually talking with the patient long enough and they will tell you every thing you need to help them with their desires. This is what homeopaths do best. Getting down to the core issues. Get the patient involved immediately as they make the appointment. Have the patient set up boundaries before starting the program. If they have a mate or friends that smoke ask them not to have them smoke around the patient for at least a week after they begin the program. Ask the patient not to carry around packs of cigarettes and if at all possible to avoid smoke during the initial detox. This is because while the body is releasing the poisonous chemicals from their system the brain and even their cellular memory is crying out for the fix of the addicting substance they are used to. Assure them that you can very easily assist them not to have cravings but that the psychological aspect has to be addressed as well. They have to avoid situations, which might jeopardize their success until they have modified their behavior sufficiently. Without their desire and effort no pill, potion or method will be effective for the rest of their life. They are the ONLY one who can control the success. The physician just helps fine-tune the skills and remedies needed for them to become a non-smoker with minimum stress or weight gain. This is part of the psychological aspect: It is also wise to set up a plan for four treatments. Some states will allow you to give a discount if they pay for all of the visits up front. Other states say you cannot be paid for services before they are given. Find out the law in your state. It is best to get the money up front for a minimum of four visits set over a three-week period. Two times the first week, then one time a week for two weeks if you are using the acupuncture/ creative visualization protocol with homeopathy. Otherwise set up a phone or check in system and have them come back once a week to listen to tapes for a month. Encourage them that at any time if they get off track they should come back in as soon as possible. Many times the patient will try to negotiate coming fewer times. The follow-up is important for both of you to know you are on track. The reinforcement is necessary. The Protocol: When you set the appointment instruct the patient: First write down a list of all of the benefits of becoming a non-smoker. Their goals and reasons. What they will do instead of smoking like exercise, breathing, walking, creative visualization, etc. They are to bring the list with them to their treatment. The physician uses this to tailor a program towards the patient’s goals and desires. It is important to reinforce their goals and reasons and if they hit a rough spot you can share these with them again. One of the benefits is saving the money they would have spent on cigarettes. In 2003, this can be $2.00- $3.00 USD a pack or more. A one pack a day smoker would be able to save between $900.00-$1100.00 dollars in a year depending on where they lived. By the way, smokers will almost always find the money to smoke. Rarely does one stop because of finances. Suggest they put the money aside and after one year of being smoke free take a vacation or do something special just for them. Make a homeopathic remedy out of that person's own brand of smoke and intention: On the night before their appointment before going to bed they are to get a very clean glass jar or bottle, preferably sterilized, or washed in the dishwasher, which they can tightly cover. Fill it 2/3 full with distilled water. As they smoke their last cigarettes have them blow the smoke into the distilled water putting their intention that it remove all the cravings and the toxins from their system. Instruct them as they smoke to: Imagine yourself in future situations when you are around others who smoke and how you can handle it without desiring to join in. If your habit is to smoke after meals or with friends on break time, come up with a game plan of a new activity you will find just as satisfying. Be peaceful with your decision and know how healthy you are. All cells are returning to the purified state before you ever were exposed to smoke. Blow all of these prayers and intentions into the smoke water. Repeat the affirmation: I release all attachments that I have to addictions. I freely choose all of my behaviors to allow me to be healthy and well. You can customize the affirmation for their personal desires as long as it is positive and in the present state. Blow all of these prayers and intentions into the smoke water. This may sound hokie to you and/or your patient. It is important that you set the right attitude that they have already decided to be a non-smoker and so that is the intention they are placing into the water. Water holds memory. This would give you a chance to explain how homeopathy works and it’s principals. Tell them you will make an individualized remedy out of this smoke water to stop their cravings. It won’t necessarily work on anyone else because the patient is putting their own intention into the remedy that you are customizing for them. Instruct them not to smoke the day of the treatment for better results. However if they are smoking as they come in the door don’t chastise them for it. Remember they are letting go of an addiction that the American Cancer Society says is as hard to break as a heroin addiction. If you have the space you can have them go outside to smoke. Create a comfortable area for them outside if possible. Give them a clean container filled with the distilled water and a straw. Compliance is important: You will be able to tell the patients commitment by the quality of homework they did before coming in for the first session. If they did not write the list of reasons they smoke and the goals for being a clean air breathing individual you might want to help them clarify their reasons for coming to you. This would be an excellent person to get all four treatments paid for as a package deal on the first visit. Perhaps money would be their motivation to get their moneys worth especially since they already paid for it. The Physicians Role:For the physical and psychological cravings make a homeopathic of their brand of tobacco smoke, potentizing it to 12C. 1 part of smoke water to 99 parts of distilled water succussed and repeated 12 times. Make sure to use a clean glass/cup and clean straw for each round of succussion. Bottle this in a one or two ounce dropper bottle and use the homeopathic that is most appropriate for the person. I use alcohol as a preservative unless the person is sensitive to it or does not desire it. Explain if they let the remedy sit in a glass of water for 5 minutes after stirring it vigorously all of the alcohol will evaporate. If they still object then I use vinegar or glycerin as a preservative. Individualizing the Remedy In customizing the homeopathic remedy for the patient it is important to identify the patient according to similimum: Arsenicum (Arsenic) (Ars.) Should be thought of in ailments from chewing tobacco; alcoholism, ptomaine poisoning, stings, dissecting wounds, Ill effects from decayed food or animal matter; odor of discharges is putrid; Thinks it useless to take medicine. Burning pains, with restlessness; with cold skin. Dyspepsia from tobacco, vinegar, acids, ice cream, ice water. Heart.-Palpitation, pain, dyspnea, faintness. Irritable heart in smokers and tobacco-chewers. Dilatation. Cyanosis. Fatty degeneration. Angina pectoris, with pain in neck and occiput. Pulse more rapid in morning. [sulph.] Fastidious, Orderly. Great anguish and restlessness. Changes place continually. Fears, of death, of being left alone. Great fear, with cold sweat. Modalities.-Worse, wet weather, after midnight; from cold, cold drinks, or food. Seashore. Right side. Better from heat; from head elevated; warm drinks. Antidotes: Opium; Carbo; China; Hepar; Nux. Chemical Antidotes: Charcoal; Hydrated Peroxide of Iron; Lime Water. Caladium Seguinum (Calad.) (American Arum) Modifies craving for tobacco. Tobacco heart. Asthmatic complaints. Headaches and mental states of smokers. Very forgetful, does not know about the occurrences of things. Confused. Desires Tobacco yet may be greatly aggravated by it. Tobacco may even cause impotence. Burning sensation in lungs after smoking (even for years after smoking just once). May also have asthma worse smoke. Nervous excitement with sensitivity to noise. Loss of interest in pleasure, joy. Emotionally 'neutral' with mental weakness. Fear and anxiety, worse evening in bed; about health Coldness of single parts and inclination to lie down, with aggravation on lying on left side. Slightest noise startles from sleep. Dread from motion. Burning sensations. Worse: Heat; 9-11 p.m. Better: Open, cold air; perspiration; short sleep especially daytime. Incompatible : Arum triph. Ignatia Amara (Ign.) (St. Ignatius Bean)Intolerant or aversion of tobacco. Tooth pain worse tobacco and coffee. Congestive headaches following anger or grief; worse, smoking or smelling tobacco, inclines head forward. The person who may have grief issues with giving up the cigarettes and have contradictory symptoms. May be more sensitive and/or romantic (the female Nux who can be just as intense but usually driven by feelings of separation, grief and depression). Unpredictable changeable emotions even hysterics, sighing. Dissatisfied with their emotional life. Modalities. Worse, in the morning, open air, after meals, coffee, smoking, liquids, external warmth. Better, while eating, change of position. Complementary: Nat. mur. Incompatible: Coffea; Nux; Tabac. Antidotes : Puls.; Cham.; Cocc. Kalmia Latifolia (Kalm) (Mountain Laurel) Neuralgia, Rheumatic, and organic heart dis-ease due to smoking Palpitation and pains worse lying left side. Better lying on back (Lil-t). Visible palpitations; loud murmurs. Pain from chest extending to left hand. Angina pectoris. Tobacco heart. Valvular heart disease (Naja).Sharp pains take away breath. Rheumatism of muscles, pain < motion (Ran-b, Spig). Weak, slow pulse (Dig). Modalities. Worse, leaning forward [opposite, Kali crab.]; looking down; motion, open air. Complementary : Benz. acid. Nux Vomica (Nux v) Poison Nut, (Quakers Button) This is probably a chain smoker. Bad taste due to tobacco. Intoxicated feeling; worse, morning, mental exertion, tobacco, alcohol, coffee, open air. Optic nerve atrophy, from habitual use of intoxicants. Paresis of ocular muscles; worse, tobacco and stimulants. Orbital twitching radiating towards the occiput, Optic neuritis. Someone who is: driven, competitive, work-a-holic, under mental strain. Loves coffee, stimulants and aggravated by them. Can be bossy and at times aggressive and irritable and CHILLY. May get palpitations after stimulants. Constipated. Inimical : Zinc. Antidotes : Coff.; Ign.; Cocc Plantago Major (Plantain) Depression and insomnia of chronic Nicotinism. Causes an aversion to tobacco. Has considerable clinical reputation in the treatment of earache, toothache, and enuresis. Sharp pain in eyes, reflex from decayed teeth or inflammation of middle ear. Eyeball very tender to touch. Pain plays between teeth and ears. Pyorrhea alveolaris. Tabacum: Desires tobacco but worse or aggravated by it. Constipation worse smoking Arteriosclerosis, High Blood Pressure and Heart problems from smoking. Usually have COLD symptoms: Stomach. Incessant nausea; worse, smell of tobacco smoke [Phos.]Symptoms with deathly nausea, with or without vomiting, icy coldness and sweat.Worse Tobacco, OPENING EYES, LEAST MOTION. Tobacco blindness. Better Fresh air, Uncovering abdomen. TERRIBLE FAINT, SINKING FEELING at pit of stomach.Nasopharyngitis and tracheitis, hemming, morning cough, sometimes with vomiting. Hoarseness of public speakers. Do not use vinegar with this remedy as it antidotes it. Also be careful of sour apples. Camphor is the physiological antagonist. Potency Selection: Use a 6C, 12C, or 30C of the similimum. Drop 1 or 2 pellets into the bottle of the desired remedy into the prepared individualized smoke remedy. Then succuss it with your own intentions and prayers that they be healthy, happy and successful. INTENTION is everything. Instruct the patient to:Tap the bottle hard on their hand several times then put 10 drops in an ounce of purified water. Stir vigorously with a spoon and sip slowly holding each sip in the mouth for 30-60 seconds. The first day take TID, Second day BID am and pm and third day one time in the am unless it is the Nux or the Tab and then take in the pm. After that Take only when they have an uncontrolled cravings. Explain to them that if they just take it because they have it, that they could cause a craving to come on. The opposite effect of what they want. We do not desire our patients to start proving the remedy. Again bad for your reputation. Explain how less is more in homeopathy. Tell them there are other methods to control cravings that you will also teach them. This remedy is there though when they really need it. Breathing Techniques: As an alternative to the deep relaxation technique instruct the patient to breathe in deeply into the belly while slowly counting to four … hold for the count of four… exhale to the count of four… pause to the count of four and repeat 10 times. This can be done anywhere at any time. If they desire they can repeat the affirmation, which hopefully they have memorized by now. Nutritional Support: Eat raw sunflower seeds, which will also help with the cravings. Increasing Vitamin C, B complex, and a multi vitamin should be plenty although there are other nutritional substances I recommend when warranted. Drink at least 8 glasses of purified water a day to help eliminate the toxins, unless contraindicated for medical reasons. Sip frequently throughout the day. Cut back or eliminate sugars and no artificial sweeteners for a few days because this can create craving aggravations. During the initial treatment instruct patients to refrain from smoking or having any cigarettes in their possession. If they make it through the next 24 hours smoke-free, they have a great chance of being successful. Discuss what they will do instead of taking that smoking break. Ask them to avoid contact with their smoking buddies if possible for a few days until after the cravings are completely gone. Make sure their confidence level to handle the situation is high. Have them create the satisfactory alternatives to modify their old behavior habits. At some point I tell them they will be helping the whole planet's atmosphere and every other living creature on the planet by not putting those toxins into the air that circulates over the whole planet. I ask them: Do you know the story of how the wings of a butterfly flapping in Africa affects the air in North America? We are all one and what you do affects all of us. The fallout from Chernobyl did touch the USA and the whole world eventually though in decreasing amount as it spread out farther and farther. Their decision to become a non-smoker can help keep pollutants out of our air. Another benefit is adding years and quality to their life and the lives of those nearest to them. Recent studies suggest second hand smoke may be equally as dangerous as smoking. Studies have shown that on the average a craving will only last two minutes. This would be a great time to do the relaxation technique. The breathing technique can be done anywhere anytime. If they have a desk job it could be a perfect break to become more relaxed and then productive. Anything else to focus on. Follow Up: Design a follow-up post card to remind the patient to check back in with you. Have them fill out three cards or just three mailing labels with their contact information including their email address and phone number. You pick out the time markers: three months, six months, nine months and a year. In today’s electronic time you can ask them for their email address and then only mail out the postcard if the email comes back as undeliverable to save on postage. If you are using the labels you won’t waste a postcard if you didn’t need to mail it out. Then put them in a file dated by month to remind you when each patient needs to be contacted. Follow up is important to know how your protocol is working. Just because you never see them again does not mean you were successful. PATIENT INFORMATION HANDOUTAt the initial office visit give a handout to the patient. Send them home with a page outlining the instructions for: Taking the remedy, How and when to push on the ear seeds if you are using acupuncture, How to do the creative visualization and breathing exercises. Have them write on the page (in their own handwriting for best effect) what behaviors they have decided they will replace the old ones with. Follow HALT don’t get Hungry, Angry, Lonely, or Tired because it can lead to a relapse. Have them post the paper and affirmations in a place they will see it daily. Give them several of your business cards with the affirmation on it for them to put by their bed, at work and any place else they may be. They can even share it with friends. You can offer them a free tune-up for a follow up if done on the markers you set. After the initial treatment of four times for acupuncture Offering free “tune-ups” at these times or PRN if desired will help keep the patient thinking of you and let you know how your results are coming. Encourage people to call immediately if necessary to get back on track. IT IS IMPORTANT TO REINFORCE THAT THEY ARE A NON-SMOKER AND FREE FROM ADDICTIONS NOW. THE WAY TO STAY THAT WAY IS TO NEVER PUT ANOTHER CIGARETTE IN THEIR MOUTH. THERE IS NO SUCH THING AS I CAN ONLY HAVE ONE CIGARETTE AND I WON’T GO BACK TO SMOKING. However, if they do choose to start smoking again encourage them to call you immediately to get back on track. Show them that you will not judge them for this but are there to help them. However, you might tell them that would break the contract of the free follow-up reinforcement session. This depends on your personality and rapport with your patients of course. The follow-up session will also give them the chance to say something else is bothering them and can you help them in other ways. Maybe constitutional homeopathic treatments. Design a certificate (with your name and clinic name and contact information of course) for them to hang on their wall. Getting a certificate is something you explain to them on the initial intake while reinforcing that they will be successful in becoming a non-smoker. This will reinforce how well they did and remind them and others that you are available to help as many of their friends as you can. After a year of being smoke free, if you have the space you can even have an open house for educational purposes or to celebrate people being addiction free. Using all of the above techniques combine with some hypnosis/creative visualization therapeutics and ear and body acupuncture is my preferred method. Get brochures to give them from the American Cancer Society. As a hypnotherapist I would share with them The American Cancer Society reports: Ingredients in tobacco: Cigarettes, cigars, and smokeless and pipe tobacco consist of dried tobacco leaves, as well as ingredients added for flavor and other properties. More than 4,000 individual compounds have been identified in tobacco and tobacco smoke. Among these are about 43 compounds that are carcinogens. There are hundreds of substances that are added by manufacturers to cigarettes to enhance the flavor or to make the smoking experience more pleasant. Some of the more common include ammonia, tar and carbon monoxide. You are putting these known toxins into your body with each drag and the heat from the cigarette can reach 160 degrees, which you draw directly into your lungs. Why stop smoking now? Within 20 minutes: Blood pressure, body temperature and pulse rate will drop to normal. Within eight hours: Smoker's breath disappears. Carbon monoxide level in blood drops; oxygen level rises to normal. Within 24 hours: Chance of heart attack decreases. Within 48 hours: Nerve endings start to regroup. Ability to taste and smell improves. Within three days: Breathing is easier. Within two to three months: Circulation improves. Walking becomes easier. Lung capacity increases up to 30 percent. Within one to nine months: Sinus congestion and shortness of breath decrease. Cilia that sweep debris from your lungs grow back. Energy increases. Within one year: Excess risk of coronary heart disease is half that of a smoker. Within two years: Heart attack risk drops to near normal. Within five years: Lung cancer death rate for average formaer pack-a-day smoker decreases by almost half. Stroke risk is reduced. Risk of mouth, throat, and esophageal cancer is half that of a smoker. Within 10 years: Lung cancer death rate is similar to that of a non-smoker. The precancerous cells are replaced. The above list is a short version of all the damage smoking is known to cause. It also shows you that your body will heal to the extent possible if you will only give it a chance. I would then lead them in a visualization. Imagine yourself going back in time to the time before you ever had your first drag off of a cigarette or were exposed to smoke. Remember how pure and healthy your body is. Recreate that pristine purity in the lungs and cells of the body. Now rewrite the memory of your DNA and RNA in the cellular body to restore that sense of balance and health. Every cell of your body now recreates those healthy cells and only those healthy cells. Tell the patient to remember what it is like to be healthy and able to be around smoke from others with no desire or aversion. They will be indifferent and immune from other people’s smoke. Their body only creates health from now on. Have them find a support person to help them through the tough times.
