to have been passed?

 

The main database can have the following fields:

 

1. Full Name.

2. Qualifications.

3. Specialisation.

4. Registration no.

5. Experience.

6. Full address of clinic.

7. Contact no of clinic.

8. email.

9. Any other information.

10.Treatment will be my own responsibility. There is no connection

between my personal treatment with the ayurveda group.

 

The database of patient who has benefitted by holistic healer.

 

1. Name of Doctor.

2. Specialisation.

3. Address of Doctor.

4. How benefitted.
